MRI对胆囊良性与恶性病变的鉴别诊断效果分析

摘　　要：目的:探讨MRI对胆囊良性与恶性病变的鉴别诊断效果及价值。方法:选取2018年12月~2020年12月本院接收的29例恶性胆囊病变患者与43例良性胆囊病变患者,分别对其开展MRI检查,通过对不同病变患者的检查结果进行比较来评价MRI在胆囊良性与恶性病变中的鉴别诊断效果,并以Kappa检验判定其诊断的一致性。结果:72例患者的MRI检查显示良性41例,恶性31例,阳性预测值为87.1%,阴性预测值为95.1%,灵敏度为93.1%,特异度为90.7%,诊断一致性为91.7%,MRI检查与病理诊断结果的一致性Kappa值为0.827,具有较好的一致性。结论:MRI有助于鉴别胆囊良性病变与恶性病变,与实验室病理诊断结果具有较高的一致性,适宜使用在胆囊良恶性病变的鉴别诊断中。Objective:To explore the differential diagnosis effect and value of MRI in benign and malignant gallbladder diseases.Methods:29 patients with malignant gallbladder diseases and 43 patients with benign gallbladder diseases received in our hospital from December 2018 to December 2020 were examined by MRI.The differential diagnosis effect of MRI in benign and malignant gallbladder diseases was evaluated by comparing the examination results of different patients,and the consistency of diagnosis was determined by Kappa test.Results:MRI examination of 72 patients showed that 41 cases were benign and 31 cases were malignant,with positive predictive value of 87.1%,negative predictive value of 95.1%,sensitivity of 93.1%,specificity of 90.7%,and diagnostic consistency of 91.7%.The Kappa value of the consistency between MRI examination and pathological diagnosis results was 0.827,showing good consistency.Conclusion:MRI is helpful to differentiate benign and malignant gallbladder lesions,which is consistent with laboratory pathological diagnosis results,and is suitable for differential diagnosis of benign and malignant gallbladder lesions.
