摘 要:目的:探讨无创正压通气联合呼吸机高压氧供给在慢阻肺干预中的应用及对患者肺功能影响。方法:将2019年12月~2020年12月本院收治的100例慢阻肺患者随机分为研究组和对照组,各50例;研究组采用无创正压通气联合呼吸机高压氧供给,对照组采用单一无创正压通气。比较两组干预前后的血气指标、肺功能指标以及生命质量评分变化。结果:研究组干预后pH、PaO_(2)、SaO_(2)均高于干预前,PaCO_(2)低于干预前(P<0.05)。对照组干预后pH值与干预前差异不明显(P>0.05),PaO_(2)、SaO_(2)高于干预前,PaCO_(2)低于干预前(P<0.05)。干预后,研究组pH、PaO_(2)、SaO_(2)高于对照组,PaCO_(2)低于对照组(P<0.05)。两组干预后VC%、FEV_(1)、FEV_(1)/FVC均高于干预前,且研究组高于对照组(P<0.05)。干预后,研究组患者生活质量评分高于对照组(P<0.05)。结论:慢阻肺患者采用无创正压通气联合呼吸机高压氧供给可以有效改善患者血气指标,提升肺功能及生命质量,具有一定的应用价值。Objective:To explore the application of noninvasive positive pressure ventilation combined with breathing and hyperbaric oxygen supply in the intervention of COPD and its effect on lung function.Methods:100 patients with COPD in our hospital from December 2019 to December 2020 were randomly divided into study group and control group,50 cases in each group.The study group was treated with noninvasive positive pressure ventilation combined with ventilator hyperbaric oxygen supply,and the control group was treated with single noninvasive positive pressure ventilation.The changes of blood gas index,pulmonary function index and quality of life score were compared before and after intervention.Results:The pH,PaO_(2) and SaO_(2) of the study group after intervention were higher than those before intervention,and PaCO_(2) was lower than that before intervention(P<0.05).The pH of the control group after intervention was not significantly different from that before intervention(P>0.05),while PaO_(2) and SaO_(2) were higher than those before intervention,and PaCO_(2) was lower than that before intervention(P<0.05).After the intervention,pH,PaO_(2) and SaO_(2) in the study group were higher than those in the control group,while PaCO_(2) in the study group was lower than that in the control group(P<0.05).After the intervention,VC%,FEV_(1) and FEV_(1)/FVC of the two groups were higher than before,and the study group was higher than the control group(P<0.05).After the intervention,the quality of life score of the study group was higher than that of the control group(P<0.05).Conclusion:Noninvasive positive pressure ventilation combined with ventilator hyperbaric oxygen supply can effectively improve the blood gas index,lung function and quality of life of patients with COPD,which has a certain application value.
