摘　　要：目的探讨沙库巴曲缬沙坦对扩张型心肌病心力衰竭患者心脏功能和心理状态的影响。方法60例扩张型心肌病心力衰竭患者,随机分为研究组与对照组,各30例。研究组应用沙库巴曲缬沙坦治疗,对照组应用缬沙坦治疗。比较两组治疗前后的心脏功能指标、躯体化症状自评量表(SSS)评分。结果治疗后,两组血清N末端-脑钠肽前体(NT-proBNP)低于治疗前,且研究组(280±22.17)pg/ml低于对照组的(536±31.09)pg/ml;两组6 min步行试验(6-MWT)长于治疗前,且研究组(447±28.48)m长于对照组的(266±32.32)m;两组左心室射血分数(LVEF)高于治疗前,且研究组(51.31±5.46)%高于对照组的(43.79±5.31)%;两组左心室舒张末期内径(LVEDD)小于治疗前,且研究组(50.34±5.37)mm小于对照组的(58.01±5..26)mm;差异均有统计学意义(P<0.05)。治疗后,两组SSS评分均较治疗前降低,且研究组(24.12±2.07)分低于对照组的(35.68±3.01)分,差异有统计学意义(P<0.05)。结论沙库巴曲缬沙坦可以显著改善扩张型心肌病心力衰竭患者心脏功能及焦虑心理状态。Objective To discuss the effect of sacubitril-valsartan on cardiac function and psychological state of patients with dilated cardiomyopathy and heart failure.Methods A total of 60 patients with dilated cardiomyopathy and heart failure were randomly divided into the research group and the control group,with 30 cases in each group.The research group was treated with sacubitril-valsartan,and the control group was treated with valsartan.The cardiac function and self-rating somatization symptom scale(SSS)score were compared between the two groups before and after treatment.Results After treatment,the serum N-terminal pro-brain natriuretic peptide(NT-proBNP)in both groups was lower than those before treatment in this group,and the research group[(280±22.17)pg/ml]was lower than the control group[(536±31.09)pg/ml];the 6-min walking test(6-MWT)in the two groups was longer than that before treatment in this group,and the research group[(447±28.48)m]was longer than the control group[(266±32.32)m];the left ventricular ejection fraction(LVEF)in the two groups was higher than that before treatment in this group,and the research group[(51.31±5.46)%]was higher than the control group[(43.79±5.31)%];the left ventricular end-diastolic diameter(LVEDD)in the two groups was smaller than that before treatment in this group,and the research group[(50.34±5.37)mm]was smaller than the control group[(58.01±5.26)mm];all the differences were statistically significant(P<0.05).After treatment,the SSS scores of the two groups were lower than those of this group before treatment,and the research group[(24.12±2.07)points]was lower than the control group[(35.68±3.01)points],and the difference was statistically significant(P<0.05).Conclusion Sacubitril-valsartan can significantly improve the cardiac function and psychological state of patients with dilated cardiomyopathy and heart failure.
