甘精胰岛素联合瑞格列奈治疗老年糖尿病的疗效分析

摘　　要：目的分析治疗甘精胰岛素联合瑞格列奈治疗老年糖尿病的临床疗效。方法56例老年糖尿病患者,采用随机数字表法分为对照组和观察组,每组28例。对照组患者给予甘精胰岛素治疗,观察组在使用甘精胰岛素治疗基础上给予瑞格列奈治疗。比较两组患者治疗前后血糖指标[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)]水平、治疗效果、副反应发生情况。结果治疗后,两组患者FPG、2 h PG、HbA1c水平均低于治疗前,且观察组患者FPG(7.61±1.20)mmol/L、2 h PG(9.34±2.24)mmol/L、HbA1c(6.12±2.59)%均低于对照组的(9.25±2.21)mmol/L、(11.35±2.29)mmol/L、(8.32±1.91)%,差异均具有统计学意义(P<0.05)。观察组患者治疗显效17例,有效10例,无效1例;对照组患者治疗显效11例,有效9例,无效8例。观察组治疗总有效率96.43%高于对照组的71.43%,差异具有统计学意义(χ2=6.487,P=0.011<0.05)。两组患者均无严重副反应。结论在甘精胰岛素基础上加用瑞格列奈治疗老年糖尿病效果确切,可更好改善机体血糖水平,且用药安全,具有临床推广价值。Objective To analyze the clinical efficacy of insulin glargine combined with repaglinide in elderly patients with diabetes mellitus.Methods A total of 56 elderly patients with diabetes mellitus were divided into control group and observation group by random numerical table,with 28 cases in each group.The control group was treated with insulin glargine,and the observation group was treated with repaglinide on the basis of insulin glargine therapy.The levels of blood glucose indexes[fasting plasma glucose(FPG),2-h postprandial glucose(2 h PG),glycosylated hemoglobin(HbA1c)]before and after treatment,therapeutic effects,and side reactions were compared between the two groups.Results After treatment,the levels of FPG,2 h PG,and HbA1c in the two groups were lower than those before treatment in this group,and the FPG(7.61±1.20)mmol/L,2 h PG(9.34±2.24)mmol/L,and HbA1c(6.12±2.59)%in the observation group were higher than(9.25±2.21)mmol/L,(11.35±2.29)mmol/L,and(8.32±1.91)%in the control group.All the differences were statistically significant(P<0.05).In the observation group,17 cases were markedly effective,10 cases were effective,and 1 case was ineffective;in the control group,11 cases were markedly effective,9 cases were effective,and 8 cases were ineffective.The total effective rate 96.43%in the observation group was higher than 71.43%in the control group,and the difference was statistically significant(χ2=6.487,P=0.011<0.05).No serious side reactions occurred in either group.Conclusion Insulin glargine combined with repaglinide is effective in the treatment of senile diabetes mellitus,which can better improve the blood glucose level and is safe to use,which has the value of clinical promotion.
