摘　　要：目的:探讨妊娠期高血压疾病(HDP)对双胎儿出生身长(BL)差异的影响。方法:选取2011年1月-2020年1月广州医科大学附属第三医院分娩的917例孕产妇及1834例新生儿进行回顾性分析,根据孕母是否合并HDP分为观察组和对照组。HDP孕母及分娩的新生儿为观察组,孕母418例,新生儿836例;健康孕母及分娩的新生儿为对照组,孕母499例,新生儿998例。收集两组孕母及新生儿情况,分析孕母患HDP对双胎出生身长的影响。结果:观察组孕母年龄高于对照组,差异有统计学意义(P<0.05);观察组新生儿出生胎龄小于对照组,且出生胎龄<34周比例大于对照组,差异有统计学意义(P<0.05)。按双胎BL差百分比≥10%、≥15%分别作为双胎出生身长不一致(BLDT)的标准,观察组BLDT发生率均高于对照组,差异有统计学意义(P<0.05)。结论:HDP可影响双胎儿宫内体格生长,导致双胎儿BL差异程度增高。Objective:To explore the effect of hypertensive disorders in pregnancy(HDP)on the birth length(BL)difference in twins.Methods:From January 2011 to January 2020,a total of 917 pregnant women and 1834 neonates in the Third Affiliated Hospital of Guangzhou Medical University from January 2011 to January 2020 were selected for retrospective analysis,and they were divided into observation group and control group according to whether the pregnant mothers had HDP.HDP pregnant mothers and their newborns were included as the observation group,with 418 pregnant mothers and 836 newborns.Healthy pregnant mothers and their newborns were included as the control group,with 499 pregnant mothers and 998 newborns.The conditions of pregnant mothers and neonates in the two groups were collected,and the effect of maternal HDP on the birth length of twins was analyzed.Results:The maternal age of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05);the birth gestational age of the observation group was lower than that of the control group,and the proportion of birth gestational age<34 weeks was greater than that of the control group,and the difference was statistically significant(P<0.05).According to the twin BL difference percentage≥10%and≥15%as the standard of twin birth length discordance(BLDT),the incidence of BLDT in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:HDP can affect the intrauterine physique growth of twin fetuses,resulting in an increased degree of BL difference between twin fetuses.
