三联法治疗湿热下注型肛门瘙痒症的疗效观察

摘　　要：目的:分析内服、外洗、火针三联法治疗湿热下注型肛门瘙痒症的临床疗效。方法:选取2018年5月-2021年9月玉溪市中医医院门诊治疗的肛门瘙痒症患者120例为观察对象,随机数字表法分为观察组与对照组,各60例。对照组采用高锰酸钾1∶5000 mL坐浴,口服氯雷他定片,复方酮康唑乳膏涂擦患处,1次/d,观察组采用内服自拟祛湿止痒方,3次/d,1剂/d;外用中药苦参汤外洗,先熏后洗,1次/d,1剂4 d;针刺阿是穴。观察两组术前、术后不同时间肛门瘙痒、肛门潮湿、肛门疼痛、皮损面积评分,临床疗效,患者满意度及复发情况。结果:治疗1周和治疗2个月,观察组肛门瘙痒评分、肛门潮湿评分、肛门疼痛评分、皮损面积下降程度、总有效率、患者满意度高于对照组,差异均有统计学意义(P<0.05)。结论:内服、外洗、火针三联法治疗湿热下注型肛门瘙痒症的临床疗效显著。Objective:To analyze the clinical effect of internal administration,external washing and fire needle triple therapy on the treatment of pruritus ani with syndrome of downward diffusion of damp-heat.Methods:A total of 120 patients with pruritus ani who were treated in the Outpatient Department of Yuxi Municipal Hospital of Traditional Chinese Medicine from May 2018 to September 2021 were selected as the observation subjects,and the patients were randomly divided into the observation group and the control group,wth 60 cases in each group.The control group received potassium permanganate 1∶5000 ml sitz bath,oral loratadine tablets,and compound ketoconazole cream applied to the affected area,once per day.The observation group was given self-designed Qushi antipruritus formula for oral administration with three times per da and a dose per day,external application of Traditional Chinese medicine Sophora Sophora decoction for external washing,smoked first and then washed,once per day and one dose in four days,and combined with acupuncture at Ashi point.The scores of pruritus ani,anal dampness,anal pain and skin lesion area,the clinical efficacy,patient satisfaction and the recurrence were observed before and after operation in the two groups.Results:After 1 week and 2 months of treatment,the pruritus ani score,anal dampness score,anal pain score,decreased degree of skin lesion area,total effective rate and patient satisfaction of the patients in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The internal administration,external washing and fire needle triple therapy has significant clinical efficacy in the treatment of pruritus ani with syndrome of downward diffusion of damp-heat.
