序贯试验法测定苯磺酸瑞马唑仑在女性患者致意识消失的半数有效量

摘　　要：目的测定苯磺酸瑞马唑仑(瑞马唑仑)在女性患者致意识消失的ED_(50)。方法选择择期拟在全凭静脉麻醉下行宫腔镜检查的妇科患者32例,ASA分级Ⅰ、Ⅱ级,年龄35~55岁。麻醉诱导时应用序贯试验法给予0.10、0.13、0.16、0.20、0.25 mg/kg五种剂量之一的瑞马唑仑,采用Probit回归分析计算瑞马唑仑在女性患者致意识消失的ED_(50)和95%有效量(95%effec⁃tive dose,ED_(95))以及各自的95%CI。结果瑞马唑仑在女性患者致意识消失的ED_(50)为0.175 mg/kg(95%CI 0.153~0.206 mg/kg),ED_(95)为0.255 mg/kg(95%CI 0.213~0.577 mg/kg)。瑞马唑仑给药前后患者呼吸循环平稳,其中两例患者在给药后出现呃逆现象。结论序贯试验法测定瑞马唑仑在女性患者致意识消失的ED_(50)为0.175 mg/kg。Objective To determine the median effective dose(ED_(50))of remimazolam besylate in female patients with loss of consciousness.Methods A total of 32 women,American Society of Anesthesiologists(ASA)gradeⅠ/Ⅱ,aged 35‒55 years old,who were scheduled to undergo hysteroscopy under total intravenous anesthesia were selected.During the induction of anesthesia,the pa⁃tients were administered with remazolam besylate at one of the following five doses,0.10,0.13,0.16,0.20 mg/kg and 0.25 mg/kg in a se⁃quential test.Then,ED_(50)and 95%effective dose(ED_(95))of remimazolam besylate,and corresponding 95%confidence intervals(CI)in female patients that caused unconsciousness were calculated by Probit regression analysis.Results The ED_(50)of remimazolam besyl⁃ate in female patients that caused loss of consciousness was 0.175(95%CI 0.153,0.206)mg/kg,and ED_(95)was 0.255(95%CI 0.213,0.577)mg/kg.The breathing cycle was stable before and after administration of remimazolam besylate,and two patients had hiccups af⁃ter administration.Conclusion According to the sequential test method,the ED_(50)of remimazolam besylate that caused loss of con⁃sciousness in female patients is 0.175 mg/kg.
