摘 要:目的探讨系统性硬化病(SSc)患者血清长链非编码RNA(lncRNA)人母系表达基因3(MEG3)表达水平及其与外周血辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的关系。方法选取2018年1月至2021年3月该院收治的54例SSc患者设为SSc组,并纳入同期54例体检健康者进行对照研究(对照组)。采用实时荧光定量PCR(RT-qPCR)法检测lncRNA MEG3、维甲酸相关孤核受体γt(RORγt)mRNA、微小RNA-21(miR-21)、叉头/翼状螺旋转录因子3(Foxp3)mRNA表达水平,流式细胞仪测定Treg、Th17细胞百分比并计算Th17/Treg,Pearson法分析SSc患者血清lncRNA MEG3、miR-21表达水平与外周血Th17、Treg、Th17/Treg相关性,Logistic回归分析SSc发生的影响因素。结果SSc组患者血清lncRNA MEG3(0.47±0.16)、Foxp3 mRNA(0.52±0.17)表达水平和Treg细胞百分比[(1.73±0.58)%]低于对照组[1.04±0.35、1.06±0.35、(2.44±0.81)%],差异均有统计学意义(P<0.05);miR-21(1.86±0.62)、RORγt mRNA(1.77±0.60)表达水平和Th17细胞百分比[(1.97±0.66)%]、Th17/Treg(1.14±0.42)高于对照组[1.01±0.33、1.03±0.34、(1.13±0.38)%、0.46±0.19],差异均有统计学意义(P<0.05)。SSc患者血清lncRNA MEG3表达与Th17、RORγt mRNA、Th17/Treg、miR-21及血清miR-21表达与Treg、Foxp3 mRNA均呈负相关(P<0.05),血清lncRNA MEG3表达与Treg、Foxp3 mRNA及血清miR-21表达与Th17、RORγt mRNA、Th17/Treg均呈正相关(P<0.05)。SSc患者血清lncRNA MEG3表达水平与miR-21呈负相关(P<0.05)。lncRNA MEG3是影响SSc发生的保护因素(P<0.05),miR-21、Th17/Treg是影响SSc发生的危险因素(P<0.05)。结论SSc患者血清lncRNA MEG3表达水平较低,且与miR-21、Th17/Treg平衡显著相关,lncRNA MEG3可能是诊断SSc的潜在靶标之一。Objective To investigate the expression level of long non-coding RNA(lncRNA)human maternally expressed gene 3(MEG3)in the serum of patients with systemic sclerosis(SSc)and its relationship with peripheral blood T helper cell 17(Th17)/regulatory T cells(Treg)balance.Methods Fifty-four cases of SSc patients admitted to our hospital from January 2018 to March 2021 were selected for the study(SSc group),and 54 healthy subjects during the same period were included in the control study(control group).Real-time fluorescent quantitative PCR(RT-qPCR)method was used to detect the expression levels of lncRNA MEG3,retinoic acid-related orphan nuclear receptorγt(RORγt)mRNA,microRNA-21(miR-21),forkhead/winged spiral transcription factor 3(Foxp3)mRNA;flow cytometry was used to measure the percentages of Treg and Th17 cells,and calculate Th17/Treg;Pearson method was used to analyze the correlation between the expression levels of serum lncRNA MEG3,miR-21 and peripheral blood Th17,Treg,Th17/Treg in SSc patients;Logistic regression was used to analyze the influencing factors of SSc.Results The expression levels of serum lncRNA MEG3(0.47±0.16),Foxp3 mRNA(0.52±0.17)and the percentage of Treg cells[(1.73±0.58)%]in the SSc group were lower than those in the control group[1.04±0.35,1.06±0.35,(2.44±0.81)%],and the differences were statistically significant(P<0.05),the expression levels of miR-21(1.86±0.62),RORγt mRNA(1.77±0.60),the percentage of Th17 cells[(1.97±0.66)%]and Th17/Treg(1.14±0.42)were higher than those in the control group[1.01±0.33,1.03±0.34,(1.13±0.38)%,0.46±0.19],and the differences were statistically significant(P<0.05).The expression level of serum lncRNA MEG3 in SSc patients negatively correlated with Th17,RORγt mRNA,Th17/Treg,miR-21,and the expression level of serum miR-21 negatively correlated with Treg and Foxp3 mRNA(P<0.05).The expression level of serum lncRNA MEG3 positively correlated with Treg,Foxp3 mRNA,and the expression level of serum miR-21 positively correlated with Th17,RORγt mRNA,Th17/
