泻肺化痰方联合常规治疗对支气管扩张症急性加重期患者的临床疗效

摘　　要：目的探讨泻肺化痰方联合常规治疗对支气管扩张症急性加重期患者的临床疗效。方法80例患者随机分为对照组和观察组,每组40例,对照组给予常规治疗(体位痰液引流、拍背、盐酸左氧氟沙星等),观察组在对照组基础上加用泻肺化痰方,疗程14 d。检测临床疗效、中医证候评分、血气分析指标(PaO_(2)、PaCO_(2)、SaO_(2))、炎症因子(CRP、IL⁃6、TNF⁃α)水平、细胞免疫功能指标(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))、不良反应发生率变化。结果观察组总有效率高于对照组(P<0.05)。治疗后,2组中医证候评分、PaCO_(2)、炎症因子水平降低(P<0.05),PaO_(2)、SaO_(2)升高(P<0.05),以观察组更明显(P<0.05);观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)细胞比例升高(P<0.05),CD8^(+)降低(P<0.05),比对照组更明显(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论泻肺化痰方联合常规治疗可安全有效地改善支气管扩张症急性加重期患者临床疗效,其机制可能与调节炎症因子水平、免疫功能有关。AIM To explore the clinical effects of Xiefei Huatan Decoction combined with conventional treatment on patients with acute exacerbation of bronchodilation.METHODS Eighty patients were randomly assigned into control group(40 cases)for fourteen⁃day intervention of conventional treatment(postural sputum drainage,backslapping,levofloxacin hydrochloride,etc.),and observation group(40 cases)for fourteen⁃day intervention of both Xiefei Huatan Decoction and conventional treatment.The changes in clinical efficacy,TCM syndrome score,blood gas analysis indices(PaO_(2),PaCO_(2),SaO_(2)),inflammatory factor(CRP,IL⁃6,TNF⁃α)levels,cellular immune function indices(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and incidence of adverse reactions were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased TCM syndrome score,PaCO_(2),inflammatory factor levels(P<0.05),and increased PaO_(2),SaO_(2)(P<0.05),especially for the observation group(P<0.05);elevated the proportion of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)cells(P<0.05),and reduced the proportion of CD8^(+)cells(P<0.05)were observable in the observation group(P<0.05),which were more obvious than those in the control group(P<0.05).No significant difference in incidence of adverse reactions was found between the two groups(P>0.05).CONCLUSION For the patients with acute exacerbation of bronchodilation,Xiefei Huatan Decoction combined with conventional treatment can safely and effectively improve clinical efficacy,whose mechanisms may contribute to the regulation of inflammatory factor levels and immune functions.
