某院在推进门急诊处方前置审核系统过程中的问题与分析

摘　　要：目的推进处方前置审核系统上线工作,促进合理用药。方法运用PDCA循环管理方法,推进及完善处方前置审核系统上线工作。结果在第1次PDCA循环后,心血管科门诊处方合格率由2019年8月的75.5%提高到2019年11月的90.4%;在第2次PDCA循环后,超适应证方面系统审核不合理率由2020年1月的10.80%降低至2020年7月的4.02%;在第3次PDCA循环后相互作用方面系统审核不合理率由2020年4月的11.86%降低至2020年9月的0.09%,药师干预率由0.61%提高到23.53%。结论PDCA循环管理方法能有效推进并且完善该院处方前置审核系统,有效拦截干预不合理处方,提高审方质量,促进患者合理用药。Objective To put the prescription pre-review system online and promote rational drug use.Methods PDCA cycle management method was used to promote and improve the prescription pre-review system.Results After the first PDCA cycle,the qualified rate of outpatient prescriptions in cardiovascular department increased from 75.5%in August 2019 to 90.4% in November 2019.After the second PDCA cycle,the improper rate of system audit for off-label indications was reduced from 10.80% in January 2020 to 4.02% in July 2020.After the third PDCA cycle in terms of interaction,the irrationality rate of system audit decreased from 11.86% in April 2020 to 0.09% in September 2020,and the intervention rate of pharmacists increased from 0.61% to 23.53%.Conclusion PDCA cycle management method can effectively promote and improve the prescription pre-review system in our hospital,effectively intercept and intervene unreasonable prescriptions,improve the quality of prescription review,and promote rational drug use in patients.
