伏立康唑治疗肺部侵袭性光滑假丝酵母菌感染的疗效分析

摘　　要：目的初步探究伏立康唑与氟康唑治疗肺部侵袭性光滑假丝酵母菌感染效果的差异及对患者肝肾功能指标的影响。方法选择于2018年1月~2021年6月住院治疗的肺部侵袭性光滑假丝酵母菌感染患者64例,以随机数表法将纳入患者分为伏立康唑组与氟康唑组各32例,分别采用相应药物治疗2周,于治疗前后检测两组患者肺功能及肝肾功能指标。结果两组患者治疗前肺功能指标FEV_(1)%、FEV_(1)/FVC、FVC%,以及肝肾功能指标BUN、AST、ALT、TBil的比较无显著差异(P>0.05),而治疗2周后伏立康唑组患者肺功能指标均高于氟康唑组患者,且肝肾功能指标水平更低(P<0.05)。此外较之于氟康唑抗感染,伏立康唑可进一步降低治疗后G实验测定值水平和痰真菌培养的光滑假丝酵母菌阳性率。结论采用伏立康唑治疗肺部侵袭性光滑假丝酵母菌感染可有效改善患者肺功能,且对肝肾功能影响较小,具有更高的安全性。Objective To explore the difference of efficacy between voriconazole and fluconazole in the treatment of pulmonary invasive Candida glabrata infection and the effect on liver and kidney function indexes of patients.Methods 64 cases of patients with pulmonary invasive Candida glabrata infection treated in our hospital from January 2018 to June 2021 were divided into voriconazole and fluconazole group with random indicator method,32 cases in each group,and all the patients were with corresponding drug treatment for 2 weeks.The pulmonary function and liver and kidney function were in the detection of two groups of patients before and after the treatment.Results Before treatment,there were no significant differences in lung function indexes of FEV_(1)%,FEV_(1)/FVC and FVC%,as well as liver and kidney function indexes of BUN,AST,ALT and TBil between two groups(P>0.05).After 2 weeks of treatment,lung function indexes in voriconazole group were higher than those in fluconazole group.The levels of liver and kidney function indexes were better(P<0.05).In addition,compared with fluconazole,voriconazole could further reduce the level of G test and the positive rate of candida glabra in sputum fungal culture after treatment.Conclusion Voriconazole in the treatment of pulmonary invasive Candida glabrata infection can effectively improve lung function,and has little effect on liver and kidney function,with higher safety.
