摘 要:目的探讨妊娠期糖尿病(gestational diabetes,GDM)患者体内胰腺衍生因子(PANcreatic DERived factor,PANDER)、胎盘生长因子(PLGF)、鸢尾素(Irisin)、血清游离甲状腺素(FT_(4))的表达水平以及相关性研究。方法选取本院定期产检孕妇212例。根据口服葡萄糖耐量试验(OGTT)确诊为妊娠期糖尿病孕妇(GDM组)106例和正常孕妇(NC组)106例。抽血检测两组人群血清中PANDER,PLGF,Irisin,FT_(4)表达水平,以及与妊娠期糖尿病的相关性分析。结果妊娠期糖尿病组血清中PANDER和PLGF水平显著高于对照组,Irisin和FT_(4)水平明显低于对照组。血清Irisin和FT_(4)水平与空腹血糖(FBG)成负相关,PANDER、PLGF水平与FBG成正相关。血清Irisin和FT_(4)水平与胰岛素功能指数HOMA-β成正相关,PANDER、PLGF水平与胰岛素功能指数HOMA-β成负相关。血清Irisin和FT_(4)水平与胰岛素抵抗指数HOMA-IR成负相关,PANDER、PLGF与胰岛素抵抗指数HOMA-IR成正相关。结论PANDER,PLGF,Irisin,FT_(4)在妊娠期糖尿病患者中变化显著,其可能在GDM的发生发展中发挥着了重要作用,对GDM的诊断和预后可能具有重要价值,有希望成为新的血清炎症标志物。Objective To investigate the expression of PANDER,PLGF,FT_(4) in patients with GDM and their correlation.Methods 212 pregnant women were selected for regular maternity examination in our hospital.According to oral glucose tolerance test(OGTT),106 pregnant women with gestational diabetes mellitus(GDM group)and 106 normal pregnant women(NC group)were diagnosed.The expression levels of PANDER,PLGF,Irisin and FT_(4) in serum of the two groups were detected by blood sampling,and the correlation with gestational diabetes mellitus was analyzed.Results The serum levels of PANDER and PLGF in gestational diabetes mellitus group were significantly higher than those in control group,and the levels of Irisin and FT_(4) were significantly lower than those in control group.Serum levels of Irisin and FT_(4) were negatively correlated with fasting blood glucose(FBG),while levels of PANDER and PLGF were positively correlated with FBG.Serum Irisin and FT_(4) levels were positive correlation with HOMA-β,while levels of PANDER and PLGF were negative correlated with insulin function index HOMA-β.Serum Irisin and FT_(4) levels were negatively correlated with HOMA-IR,while PANDER and PLGF were positively correlated with HOMA-IR.Conclusion PANDER,PLGF,Irisin and FT_(4) change significantly in patients with gestational diabetes mellitus.It may play an important role in the development of GDM.It may be of great value in the diagnosis and prognosis of GDM,and is expected to become a new marker of serum inflammation.
