超声导引下PICC置管术在神经外科的临床应用及效果

摘　　要：目的:分析超声导引下在神经外科应用PICC(经外周静脉置入中心静脉导管)置管术的临床效果.方法:对象以58例神经外科住院患者为例,时间以2017年1月2019年12月为起止,回顾性分析其全部的临床资料,其中女性22例、男性36例,年龄区间22-75岁,统计均值(43.2±3.5)岁.所有对象均接受超声引导下建立静脉通路,行PICC置管操作.结果:本组58例患者均成功穿刺,成功率100.0%.置管时间区间为7d-12个月,统计均值(1.2±0.5)个月.结束治疗后患者均成功拔管.置管期间均未发生严重并发症,如静脉炎,气胸等,仅1例发生血栓,并发症率1.72%,而经抗凝治疗后效果显著,血栓消失.结论:超声导引下在神经外科应用PICC置管术的效果显著,且具有给药迅速、刺激性低、疗效快等优势,尤其是能有效抢救危重患者,此种置管术还可减少各相关并发症,如静脉炎、静脉硬化等,可降低血管刺激性,安全可靠,可做进一步推广.Objective To analyze the clinical ffects of ultrasound-guided insertion of central venous catheter(PICC)in neurosurgery.Methods:A total of 58 inpatients in neurosurgery department from January 2017 to December 2019 were included in the retrospective analysis of the clinical data,including 22 females and 36 males,with the age ranging from 22 to 75 years old,and the satistical mean value of(43.2 3.5)years old.All the subjects received ultrasound-guided establishment of venous access and underwent PICC catheterization.Results All the 58 cases were sccssfully punctured with a success rate of 100.0%.The time interval for catheterization was from 7 d to 12 months,with a statistical mean of(1.2 0.5)months.The tubes were scsfully removed after the treatment.During catheterization,no serious complications occurred,such as phlebitis and pneumothorax.Thrombus occurred in only one case,with the complication rate of 1.72%.However,after anticoagulation treaument,the effect was significant,and the thrombus disappeared.Conclusion:Ultrasound-guided PICC catheterizaion in ncurosurgery has a significant effect.Morcover,it has the advantages of rapid drug administration,low irntation,and rapid curative effect.In particular,it can effectively rescue cnitical patients.Such catheterization can also reduce various related complications,such as phlebiris and venous sclerosis,and reduce vascular rritation.It is safe and reliable,and can be further promoted.
