我国日间手术资源配置公平性及效率研究

摘　　要：目的:分析日间手术资源配置的公平性及效率,发现日间手术资源配置现状中存在的问题,为相关部门合理制定卫生资源规划、提高日间手术资源配置的公平性提供参考依据。方法:采用描述性分析、基尼系数和泰尔指数对日间手术资源公平性进行评价;采用运用数据包络分析法从投入和产出两个维度出发,对日间手术资源的配置效率进行分析。结果:2019年,东部地区日间手术资源的总体配置遥遥领先,其次是中部地区和西部地区;按地区人口分布的日间手术床位资源基尼系数为0.1595<0.2,日间手术人力资源基尼系数为0.1504<0.2,两类资源总体分布均处于较公平的状态;从泰尔指数分布来看,日间手术医疗资源的配置存在地区间差异大于地区内部差异的特点;从平均配置效率来看,东、中、西3个地区间日间手术资源的配置效率存在明显的差异性。结论:各省日间手术资源总体配置较公平但地区间差异较大,地区间日间手术资源配置效率存在差异性,且各省间配置效率仍处于较低水平。因此,需要综合考虑区域卫生资源规划、经济水平、人口学特征、疾病谱等基本情况,因地制宜适当扩大或压缩日间手术资源规模,优化资源投入规模与结构,最大限度利用日间手术资源。Objective:To analyze the fairness and efficiency of the allocation of day surgery resources,find out the prominent problems in the current situation in China,and provide references for relevant departments to make reasonable health resource planning and improve the fairness of the allocation.Methods:Descriptive analysis,Gini coefficient and Theil index were used to evaluate the equity of day surgery resources in China.Data envelopment analysis was used to analyze the allocation efficiency of day surgery resources from two dimensions of input and output.Results:The resources for day surgery in China were the highest in the eastern region,followed by the central region,and finally the western region.The Gini coefficient of the resources of the day surgery beds and the human resources were 0.1595<0.2 and 0.1504<0.2 according to the regional population distribution in China.The overall distribution of the two kinds of resources was in a fairly fair state.In terms of the distribution of the Theil index,the distribution of medical resources for day surgery is more significant between regions than within regions.From the perspective of average allocation efficiency,there are obvious differences in the allocation efficiency of daytime surgical resources among the eastern,central and western regions of China.Conclusion:The overall allocation of day surgical resources is fair among provinces in China,but there are great differences among regions.There are differences in the allocation efficiency of day medical resources among regions,and the allocation efficiency among provinces is still at a low level.Therefore,health administrative departments need to comprehensively consider the basic situation of regional health resource planning,economic level,demographic characteristics,disease spectrum and so on,appropriately expand or compress the scale of daytime resources,optimize the scale and structure of resource input,and maximize the use of daytime medical resources.
