摘  要:目的 探讨基因芯片法联合结核分枝杆菌/利福平耐药实时荧光定量核酸扩增检测技术(Xpert MTB/RIF)对肺结核的诊断价值。方法 选取2019年3月至2021年2月本院收治的421例疑似肺结核患者为研究对象,采用基因芯片法、Xpert MTB/RIF及基因芯片法联合Xpert MTB/RIF处理痰标本。以痰培养结果为金标准,比较不同方法对肺结核的诊断效能;以药敏试验结果为参照,比较不同方法对利福平耐药的检出符合率。结果 基因芯片法联合Xpert MTB/RIF对肺结核的诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于基因芯片法、Xpert MTB/RIF,差异具有统计学意义(P<0.05)。基因芯片法联合Xpert MTB/RIF对利福平耐药的检出符合率高于基因芯片法、Xpert MTB/RIF,差异具有统计学意义(P<0.05)。结论 基因芯片法联合Xpert MTB/RIF对肺结核的诊断价值较高,且可判断结核分枝杆菌对利福平的耐药性。Objective To explore the diagnostic value of gene chip combined with GeneXpert Mycobacterium tuberculosis/Rifampin(Xpert MTB/RIF)in pulmonary tuberculosis.Methods A total of 421 patients with suspected pulmonary tuberculosis treated in our hospital from March 2019 to February 2021 were selected as the research objects.Gene chip,Xpert MTB/RIF and gene chip combined with Xpert MTB/RIF were used to treat sputum samples.Taking the sputum culture results as the gold standard,the diagnostic efficacy of different methods for pulmonary tuberculosis was compared;referring to the results of drug sensitivity test,the detection coincidence rate of different methods for rifampin-resistant was compared.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of gene chip combined with Xpert MTB/RIF in the diagnosis of pulmonary tuberculosis were higher than those of gene chip and Xpert MTB/RIF,and the differences were statistically significant(P<0.05).The coincidence rate of rifampin-resistant detected by gene chip combined with Xpert MTB/RIF was higher than that of gene chip and Xpert MTB/RIF,and the difference was statistically significant(P<0.05).Conclusion Gene chip combined with Xpert MTB/RIF has higher diagnostic value for pulmonary tuberculosis,which can judge the drug resistance of Mycobacterium tuberculosis to rifampin.
