生物反馈电刺激联合补中益气汤及针刺对Ⅰ、Ⅱ度直肠脱垂患者盆底肌功能及肛门直肠动力学的影响研究

摘　　要：目的 研究生物反馈电刺激联合补中益气汤及针刺对Ⅰ、Ⅱ度直肠脱垂患者盆底肌功能及肛门直肠动力学的影响。方法 80例Ⅰ、Ⅱ度直肠脱垂患者,随机分为对照组和观察组,各40例。对照组实施生物反馈电刺激治疗,观察组实施生物反馈电刺激联合补中益气汤及针刺治疗。比较两组患者的治疗效果、盆底肌功能、肛门功能。结果 观察组患者治疗总有效率97.50%高于对照组患者的80.00%,差异有统计学意义(P<0.05)。观察组盆底肌持续收缩时间(6.21±1.35)s、夜尿次数(1.02±0.26)次/d、阴道肌电压(21.39±4.08)μV、尿失禁次数(0.62±0.19)次/d均优于对照组的(4.19±1.39)s、(2.88±1.13)次/d、(15.74±4.31)μV、(1.47±0.36)次/d,差异有统计学意义(P<0.05)。观察组肛门坠胀评分(0.47±0.18)分、排便功能评分(3.58±1.02)分、直肠脱出程度评分(0.33±0.23)分均低于对照组的(1.03±0.67)、(8.95±2.29)、(0.87±0.39)分,差异有统计学意义(P<0.05)。结论 对Ⅰ、Ⅱ度直肠脱垂患者实施生物反馈电刺激联合补中益气汤及针刺治疗效果显著,可有效改善盆底功能及肛门功能,值得在临床中应用和推广。Objective To study the effects of biofeedback electrical stimulation combined with Buzhong Yiqi Decoction and acupuncture on pelvic floor muscle function and anorectal dynamics in patients with gradeⅠandⅡrectal prolapse.Methods A total of 80 patients with gradeⅠandⅡrectal prolapse were randomly divided into the control group and the observation group,with 40 cases in each group.The control group was treated with biofeedback electrical stimulation,while the observation group was treated with biofeedback electrical stimulation combined with Buzhong Yiqi Decoction and acupuncture.The therapeutic effect,pelvic floor muscle function and anal function of the two groups were compared.Results The total effective rate in the observation group was 97.50%,which was higher than 80.00%in the control group,and the difference was statistically significant(P<0.05).The continuous contraction time of pelvic floor muscle(6.21±1.35)s,nocturia frequency(1.02±0.26)times/d,vaginal muscle voltage(21.39±4.08)μV,and urinary incontinence frequency(0.62±0.19)times/d in the observation group were better than(4.19±1.39)s,(2.88±1.13)times/d,(15.74±4.31)μV,and(1.47±0.36)times/d in the control group,and the differences were statistically significant(P<0.05).The scores of anal bulge,defecation function,and rectal prolapse in the observation group were(0.47±0.18),(3.58±1.02)and(0.33±0.23)points,which were lower than(1.03±0.67),(8.95±2.29),and(0.87±0.39)points in the control group,and the differences were statistically significant(P<0.05).Conclusion The implementation of biofeedback electrical stimulation combined with Buzhong Yiqi Decoction and acupuncture has significant efficacy on patients with gradeⅠandⅡrectal prolapse,and can effectively improve the pelvic floor function and anal function,which is worthy of clinical application and promotion.
