摘　　要：目的探讨背侧楔形截骨术治疗第二跖骨头坏死(Freiberg病)的临床疗效。方法采用背侧楔形截骨术治疗16例Freiberg病患者。记录并发症发生情况,比较手术前后AOFAS踝-后足评分、疼痛VAS评分、跖趾关节活动度。结果患者均获得随访,时间12~33个月。切口均一期愈合,未出现转移性跖痛、跖骨头坏死等并发症。末次随访时,AOFAS踝-后足评分、疼痛VAS评分、跖趾关节活动度均较术前明显改善(P<0.01)。结论采用背侧楔形截骨术治疗Freiberg病可有效缓解跖骨疼痛症状,增加患者跖趾关节活动度。Objective To investigate the clinical efficacy of dorsal wedge osteotomy in the treatment of the nercrosis of the second metatarsal head(Freiberg disease).Methods Sixteen patients with Freiberg disease were treated with dorsal wedge osteotomy.The complications were recorded,AOFAS ankle-hindfoot score,pain VAS,the activity range of the metatarsophalangeal joint were compared before and afteRoperation.Results All cases were followed up foR12~33 months.All the incisions healed in one-stage,no metastatic metatarsalgia,metatarsal head necrosis and otheRcomplications occurred.At the last follow-up,AOFAS ankle-hindfoot score,pain VAS,the activity range of the metatarsophalangeal joint were significantly improved,compared with those before operation(P<0.01).Conclusions The dorsal wedge osteotomy can significantly relieve the symptoms of metatarsal pain,and increase the range of motion of metatarsophalangeal joint.
