摘　　要：目的了解患者及家属对不同医疗机构医生的信任状况,探索影响患者及家属对医生信任度的相关因素,为今后政策的制定提供参考。方法采用多阶段单纯随机抽样的方法,在上海市某区的一级、二级、三级医疗机构中分别抽取501名、238名和139名患者或家属为调查对象,对其人口学特征、对医疗费用的感受、患者对医生的信任状况等进行分析,并采用多元线性回归分析对影响患者或家属的医生信任度相关因素进行分析。结果调查对象中,74人(8.4%)认为当前的医疗费用很高,403人(45.9%)认为较高,312人(37.8%)认为一般,14人(1.6%)认为较低,55人(6.3%)说不清楚,不同等级医疗机构就诊的患者或家属差异有统计学意义(P<0.05);调查对象信任总得分为(24.92±4.85)分,其中仁爱平均得分(12.07±2.60)分;技术能力平均得分(12.86±2.60)分,不同等级医疗机构的得分差异均有统计学意义(P<0.05)。多元线性回归分析结果表明,工作状态、医疗机构等级是影响患者对医生信任度的相关因素。结论当前患者及家属对医生的信任度处于较低水平,认为当前医疗费用较高,改善医患之间的信任状况涉及社会多个方面,需要全社会共同努力。Objective To understand the trust status of patients and their family members in different levels of medical institutions to doctors,and explore the factors that affect the trust status of patients and their family members to doctors.Methods Using the method of multi-stage random sampling,a total of 501(57.1%),238(27.1%)and 139(15.8%)patients or family members were selected from the primary,secondary and tertiary medical institutions in a district of Shanghai city for questionnaire surveys.The survey content included demographic characteristics,attitude towards medical expenses,the patient’s trust in the doctor,etc.Multiple linear regression analysis was used to analyze the influencing factors of the trust degree of patients or their family members.Results Among the respondents,74(8.4%)thought that the current medical expenses was very high,403(45.9%)thought it was high,312(37.8%)thought it was average,14(1.6%)thought it was low,and 55(6.3%)thought it was not clear.There were significant differences in patients or family members among different levels of medical institutions;the total score of trust was(24.92±4.85),of which the average score of benevolence was(12.07±2.60),and the average score of technical ability was(12.86±2.60),the scores of different levels of medical institutions were statistically significant;the results of multiple linear regression analysis indicated that work status and medical institution level were related factors that affect patients’trust in doctors.Conclusion At present,patients and their families have a low level of trust in doctors and think that the medical expenses is high.Improving the trust between doctors and patients involves many aspects of the society,which needs the joint efforts of the whole society.
