摘　　要：目的以帕金森病患者支持服务为例,探索连续性照护的关键要素与实现机制,构建照护模型。方法采用扎根理论质性研究方法,选取与患者连续性照护相关的7组家庭、6名医疗机构工作者及4名社区机构工作者为对象进行访谈,对资料进行三级编码,构建理论模型。结果连续性照护包含三个关键要素:信息、关系及管理的连续性。信息的连续性包括信息共享、互补、整合与转介;关系的连续性包括建立稳定制度、实践全人关怀;管理的连续性强调识别连续性照护需求及提供全程健康管理服务,以此建构社会工作视角下的慢性病连续性照护模型。结论研究所构建的慢性病连续性照护模型,通过融入社会工作视角,拓展了慢性病管理的内容与形式,能够为政策制定提供参考。Objective The aim of this study is to explore the key components of continuity of care and construct continuous care model based on the case of the supportive service for patients with Parkinson’s disease.Methods Using the grounded theory method,seven families,six medical team members and four community workers were interviewed through theoretical sampling.Three-level coding was used to construct the theoretical model.Results Continuity of care includes three key elements:informational,relational and management continuity.Informational continuity includes information sharing,complementation,integration and referral;relational continuity includes stability of institutional arrangements and practice of holistic care;management continuity includes identifying needs of continuous care and providing whole-course health management package.Conclusion The continuous care model integrated with social work perspective has expanded the contents and forms of chronic disease management,and can inform policy making.
