持续气道正压通气治疗阻塞性睡眠呼吸暂停合并高血压对改善患者生命质量的效果分析

摘　　要：目的探讨持续气道正压通气治疗阻塞性睡眠呼吸暂停合并高血压对改善患者生命质量的效果。方法选取2018年5月至2020年5月本院收治的80例阻塞性睡眠呼吸暂停合并高血压患者作为研究对象,采用随机抽签的方式分为对照组与观察组,每组40例。对照组采取基础治疗措施,观察组在对照组基础上应用持续气道正压通气治疗,比较两组多导睡眠监测指标[睡眠呼吸暂停低通气指数(AHI)、血氧饱和度<80%次数、血氧饱和度<20%次数]、血压水平及生活命质量评分。结果观察组AHI、血氧饱和度<80%次数、血氧饱和度<20%次数均少于对照组,差异有统计学意义(P<0.05)。治疗后,两组收缩压、舒张压水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组生理机能、生理职能、社会功能、情感职能评分均高于对照组,差异有统计学意义(P<0.05)。结论采用持续气道正压通气治疗阻塞性睡眠呼吸暂停合并高血压患者效果显著,不仅可纠正患者的低氧血症,降低患者血压,还可改善患者生命质量,临床应用价值较高。Objective To explore the effect of continuous positive airway pressure on improving the quality of life of patients with obstructive sleep apnea complicated with hypertension.Methods 80 patients with obstructive sleep apnea complicated with hypertension who were admitted to our hospital from May 2018 to May 2020 were selected as the research subjects,and they were divided into the control group and the observation group according to random drawing,with 40 cases in each group.The control group was given basic treatment measures,the observation group was treated with continuous positive airway pressure ventilation on the basis of the control group.The polysomnography indicators(sleep apnea-hypopnea index[AHI],blood oxygen saturation<80%times,blood oxygen saturation<20%times),blood pressure levels and quality of life scores were compared between the two groups.Results The AHI,the times of blood oxygen saturation<80%,and the times of blood oxygen saturation<20%in the observation group were less than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of systolic blood pressure and diastolic blood pressure in the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The scores of physiological function,physiological function,social function and emotional function of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Continuous positive airway pressure is effective in the treatment of obstructive sleep apnea complicated with hypertension,it can not only correct hypoxemia,but also reduce blood pressure and improve the quality of life of patients and the clinical application value is high.
