关节镜下Tightrope技术与锁骨钩板治疗急性RockwoodⅤ型肩锁关节损伤的中期疗效对比

摘　　要：目的比较关节镜下Tightrope技术与锁骨钩板技术治疗RockwoodⅤ型急性肩锁关节损伤的中期疗效。方法回顾性分析2013年1月至2017年12月本院收治的43例RockwoodⅤ型肩锁关节损伤患者的临床资料,依据手术方式的不同分为A组(n=20)和B组(n=23),A组采用锁骨钩板治疗,B组采用关节镜下Tightrope技术治疗,比较两组术后并发症发生率、工作状态、Constant-Murley评分、视觉模拟评分法(VAS)评分、喙锁间隙、肩锁关节前后间距、肩关节力量。结果两组Constant-Murley评分、VAS评分、术后并发症发生率及喙锁间隙比较差异无统计学意义,两组脱离工作时间及肩关节力量恢复情况比较差异有统计学意义(P<0.05)。B组患者术后末次随访肩锁关节前后间距与健侧比较差异无统计学意义。结论治疗锁骨钩板技术或关节镜下Tightrope技术RockwoodⅤ型急性肩锁关节损伤,均可缓解患者的疼痛,恢复患肢肩关节功能,帮助患者返回最初的工作岗位。但Tightrope技术在恢复肩关节力量、恢复肩锁关节稳定性、降低并发症发生率及缩短康复时间上具有显著的优势。Objective To compare the mid-term efficacy of arthroscopic Tightrope technique and clavicle hook plate technique in the treatment of Rockwood type V acute acromioclavicular joint injury.Methods The clinical data of 43 patients with Rockwood type V acromioclavicular joint injury treated in our hospital from January 2013 to December 2017 were retrospectively analyzed,and they were divided into group A(n=20)and group B(n=23)according to different surgical methods,the group A was treated with clavicle hook plate,the group B was treated with arthroscopic Tightrope technique.The incidence of postoperative complications,work status,Constant-Murley score,visual analogue scale(VAS)score,coracoclavicular space,anterior-posterior distance of acromioclavicular joint,and shoulder joint strength were compared between the two groups.Results There was no significant difference in Constant-Murley score,VAS score,postoperative complication rate and coracoclavicular space between the two groups.There were significant differences in the time away from work and the recovery of shoulder joint strength between the two groups(P<0.05).There was no significant difference in the anterior-posterior distance of the acromioclavicular joint between group B and the healthy side at the last follow-up.Conclusion For Rockwood type V acute acromioclavicular joint injury,the clavicle hook plate technique or arthroscopic Tightrope technique can relieve the pain of the patient,restore the function of the shoulder joint of the affected limb,and help the patient return to the original job.However,the Tightrope technique has significant advantages in restoring the strength of the shoulder joint,restoring the stability of the acromioclavicular joint,reducing the incidence of complications and shortening the recovery time.
