个案管理对髋关节置换术后假体脱位的影响

摘　　要：目的探究个案管理对髋关节置换术后假体脱位的影响。方法选取2019年1—12月于本院行髋关节置换手术的70例患者作为研究对象,采用随机数字表法分为对照组(n=34)和实验组(n=36)。对照组给予常规护理,实验组在对照组基础上采用个案管理模式护理。比较两组术后假体脱位发生率、预防脱位知识了解程度评分、Harris髋关节功能评分及健康调查量表-36(SF-36)评分。结果实验组术后假体脱位发生率为2.78%,显著低于对照组的20.59%,差异有统计学意义(P<0.05)。实验组预防假体脱位知识了解程度评分[(86.78±10.76)分]、Harris髋关节功能评分[(87.56±6.05)分]、SF-36评分[(86.70±10.50)分],均高于对照组的[(66.75±9.88)、(73.85±7.84)、(73.56±8.76)分],差异有统计学意义(P<0.05)。结论个案管理能降低髋关节置换术后假体脱位的发生率,提高患者预防假体脱位知识的了解程度,改善患者的髋关节功能,提高患者的生命质量,值得临床推广应用。Objective To explore the effect of case management on prosthesis dislocation after hip replacement.Methods 70 patients who underwent hip arthroplasty in our hospital from January to December 2019 were selected as the research subjects,and they were divided into control group(n=34)and experimental group(n=36)according to random number table method.The control group was given routine nursing,and the experimental group was given case management nursing on the basis of the control group.The incidence of postoperative prosthetic dislocation,knowledge of prevention of dislocation score,Harris hip function score and 36-item short form health survey 36(SF-36)scale score were compared between the two groups.Results The incidence of prosthetic dislocation in the experimental group was 2.78%,which was significantly lower than 20.59%in the control group,the difference was statistically significant(P<0.05).The scores of knowledge of preventing prosthetic dislocation([86.78±10.76]scores),Harris hip function([87.56±6.05]scores)and SF-36([86.70±10.50]scores)in the experimental group were higher than([66.75±9.88],[73.85±7.84],[73.56±8.76]scores)in the control group,and the difference was statistically significant(P<0.05).Conclusion Case management can reduce the incidence of prosthetic dislocation after hip replacement,improve patients'knowledge of preventing prosthetic dislocation,improve patients'hip function,and improve patients'quality of life,which is worthy of clinical promotion.
