床旁超声探测颈内静脉管径、呼吸变异指数对PCAS的价值

摘　　要：目的:床旁超声探测颈内静脉管径、呼吸变异指数(RVI)对心脏骤停后综合征(PCAS)的价值。方法:以70例PCAS患者为PCAS组,80例体检健康者为对照组,超声测量颈内静脉截面积、中心静脉压(CVP),计算颈内静脉的RVI,分析上述指标之间相关性。结果:PCAS组心脏复苏上机时颈内静脉截面积大于对照组(P<0.05);PCAS组心肺复苏后1d、3d、7d颈内静脉截面积、CVP逐渐增大,RVI逐渐减小(P<0.05);PCAS组心脏复苏后1d、3d、7d颈内静脉截面积与CVP均呈正相关,RVI与CVP均呈负相关(P<0.05)。结论:床旁超声探测颈内静脉截面积与RVI可反映出CVP及PCAS患者血容量,指导液体复苏。Objective:Value of internal jugular vein diameter and respiratory variability index(RVI)measured by bedside ultrasound in post-cardiac arrest syndrome(PCAS).Methods:This study included 70 patients with PCAS,and 80 healthy individuals(control group).The cross-sectional area of internal jugular vein and central venous pressure(CVP)were measured using ultrasound.The RVI of internal jugular vein was calculated,and the correlation among these indicators was analyzed.Results:The cross-sectional area of internal jugular vein in the PCAS group when the cardiac resuscitation device was placed on the patient was larger than that in the control group(P<0.05).The cross-sectional area of internal jugular vein and CVP gradually increased,while RVI gradually decreased in the PCAS group on day 1,day 3,and day 7 after cardiac resuscitation(P<0.05).On day 1,day 3,and day 7 after cardiopulmonary resuscitation,the cross-sectional area of internal jugular vein was positively correlated with CVP,and RVI was negatively correlated with CVP(P<0.05).Conclusion:The cross-sectional area of internal jugular vein and RVI measured by bedside ultrasound can reflect the level of CVP,thereby helping to assess the blood volume of patients with PCAS and guide fluid resuscitation.
