2423例泌尿生殖道支原体感染分布情况及药敏分析

摘　　要：目的分析阳江市人民医院泌尿生殖道支原体感染分布情况及药敏情况,为临床对生殖道支原体感染诊断和治疗提供依据。方法回顾性分析7122例临床诊断为尿道炎、前列腺炎、阴道炎、宫颈炎、盆腔炎等泌尿生殖系统疾病以及不孕不育和流产早产等患者的临床资料。采用解脲脲原体(Uu)和人型支原体(Mh)培养鉴定药敏试剂盒对泌尿生殖道标本进行培养鉴定和药敏试验。比较男性与女性泌尿生殖道支原体阳性率、泌尿生殖道支原体感染类型以及3种泌尿生殖道支原体感染类型的药敏情况。结果7122例泌尿生殖道支原体培养结果,剔除同一患者复诊结果后共6567例,阳性例数为2423例,总阳性率为36.9%。其中男性阳性率为18.3%,女性阳性率为44.5%。女性的生殖道支原体阳性率高于男性,差异有统计学意义(P<0.05)。女性单纯Uu阳性率、单纯Mh阳性率和Uu+Mh混合阳性率分别为38.9%、1.0%、4.6%,均高于男性的17.5%、0.1%、0.7%,差异有统计学意义(P<0.05)。单纯Uu阳性,交沙霉素、克拉霉素、强力霉素和美满霉素敏感率达到93.6%以上,阿奇霉素敏感率达到83.6%,罗红霉素和氧氟沙星、左旋氧氟沙星、司帕沙星敏感率低于36.0%。单纯Mh阳性,强力霉素、美满霉素和交沙霉素敏感率均达到100.0%,氧氟沙星、左旋氧氟沙星、司帕沙星敏感率均低于59.6%,克拉霉素、罗红霉素、阿奇霉素耐药率均为100.0%。Uu+Mh混合阳性,强力霉素、美满霉素和交沙霉素的敏感率高于85.4%,其余药物敏感率均低于12.4%。结论阳江地区泌尿生殖道支原体感染以Uu感染为主要感染类型,强力霉素、美满霉素和交沙霉素对3种支原体感染类型有较高的药物敏感性,临床医生应该结合患者的临床症状和支原体感染类型,参考支原体培养鉴定及其药敏结果进行诊疗活动。Objective To analyze the distribution and drug sensitivity of mycoplasma infection in urogenital tract in Yangjiang People's Hospital,so as to provide basis for clinical diagnosis and treatment of mycoplasma infection in urogenital tract.Methods The clinical data of 7122 patients with urogenital diseases such as urethritis,prostatitis,vaginitis,cervicitis,and pelvic inflammatory disease,as well as infertility and premature abortion were retrospectively analyzed.Ureaplasma urealyticum(Uu)and Mycoplasma hominis(Mh)culture identification kits were used for culture,identify and test the drug sensitivity of urogenital specimens.The positive rate of mycoplasma in urogenital tract between males and females,the types of mycoplasma infection in urogenital tract,and the drug sensitivity of 3 types of mycoplasma infection in urogenital tract were compared.Results Among 7122 cases of mycoplasma urogenital tract culture,there were 6567 cases left after excluding the results of the same patient's follow-up.The number of positive cases was 2423,and the total positive rate was 36.9%,with 18.3%for males and 44.5%for females.The positive rate of mycoplasma in urogenital tract in females was higher than that in males,and the difference was statistically significant(P<0.05).The positive rates of Uu alone,Mh alone and Uu+Mh in females were 38.9%,1.0%and 4.6%,which were higher than 17.5%,0.1%and 0.7%in males,and the differences were statistically significant(P<0.05).In Uu positive patients,the sensitivity rate of josamycin,clarithromycin,doxycycline and minocycline reached>93.6%,the sensitivity rate of azithromycin reached 83.6%,the sensitivity rate of roxithromycin,ofloxacin,levofloxacin,sparfloxacin was<36.0%.In Mh positive patients,the sensitivity rates of doxycycline,minocycline and josamycin were all up to 100.0%,and the sensitivity rates of ofloxacin,levofloxacin and sparfloxacin were all lower than 59.6%,and the resistance rates of clarithromycin,roxithromycin,azithromycin rates were 100.0%.In Uu+Mh mixed positive patients,the
