摘  要:目的通过比较地中海贫血患者和健康人群的平均红细胞体积(MCV)、平均红细胞血红蛋白量(MCH)、血红蛋白F(HbF)、血红蛋白A_(2)(HbA_(2))浓度不同,研究其筛查地中海贫血患者的实际意义。方法选取14例地中海贫血患者作为观察组,另选取14例同期行健康体检的人群作为对照组。比较两组血液流变学指标、HbA_(2)检测结果分布情况。结果观察组MCV(73.01±6.16)fl、MCH(24.18±1.32)pg低于对照组的(87.54±3.75)fl、(33.12±2.47)pg,HbA_(2)(3.66±0.30)%、HbF(2.39±0.28)%高于对照组的(2.91±0.31)%、(1.78±0.14)%,差异均有统计学意义(P<0.05)。观察组HbA_(2)<2.5%占比高于对照组,2.5%≤HbA_(2)≤3.5%占比低于对照组,差异均有统计学意义(P<0.05)。两组HbA_(2)>3.5%占比均为0。结论地中海贫血患者的MCF、MCH浓度比健康人群显著下降,而HbA_(2)、HbF则比健康人群显著上升,通过上述生化筛查检验,可及时发现地中海贫血患者,为初步诊断地中海贫血提供有利依据,值得推广应用。Objective To compare the mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),hemoglobin F(HbF),and hemoglobin A_(2)(HbA_(2))concentrations between thalassemia patients and healthy subjects,and study their practical significance in screening thalassemia patients.Methods 14 cases of thalassemia patients were selected as the observation group,and 14 healthy subjects during the same period were selected as the control group.The distribution of hemorheology indexes and HbA_(2) test results were compared between the two groups.Results The MCV(73.01±6.16)fl and MCH(24.18±1.32)pg of the observation group were lower than(87.54±3.75)fl and(33.12±2.47)pg of the control group,and the HbA_(2)(3.66±0.30)%and HbF(2.39±0.28)%were higher than(2.91±0.31)%and(1.78±0.14)%of the control group.All the differences were statistically significant(P<0.05).The proportion of HbA_(2)<2.5%in the observation group was higher than that in the control group,and the proportion of 2.5%≤HbA_(2)≤3.5%was lower than that in the control group.All the differences were statistically significant(P<0.05).The proportion of HbA_(2)>3.5%in both groups was 0.Conclusion The concentrations of MCF and MCH in patients with thalassemia are significantly lower than those in healthy subjects,while the HBA_(2) and HbF are significantly higher than those in healthy subjects.Through the above biochemical screening test,patients with thalassemia can be found in time,which can provide a favorable basis for the preliminary diagnosis of thalassemia,and is worthy of popularization and application.
