托拉塞米联合多巴胺治疗老年顽固性心力衰竭的疗效观察

摘　　要：目的分析在老年顽固性心力衰竭患者治疗中应用托拉塞米联合多巴胺的临床效果。方法88例老年顽固性心力衰竭患者,依据抽签法分为实验组和对照组,各44例。对照组接受多巴胺治疗,实验组接受多巴胺联合托拉塞米治疗。比较两组治疗前后心功能指标[左心室射血分数(LVEF)、左心室收缩末期容积(ESV)、左心室舒张末期容积(EDV)]、治疗效果及治疗后不良反应发生情况。结果治疗后,实验组LVEF(41.5±2.6)%显著高于对照组的(32.5±2.2)%,ESV(100.5±1.8)ml、EDV(134.5±2.3)ml显著低于对照组的(122.5±2.1)、(164.5±1.3)ml,差异均有统计学意义(P<0.05)。实验组治疗总有效率95.45%高于对照组的70.45%,差异有统计学意义(P<0.05)。实验组不良反应发生率4.55%低于对照组的18.18%,差异有统计学意义(P<0.05)。结论采用多巴胺和托拉塞米联合治疗老年顽固性心力衰竭具有显著的临床疗效,能够显著改善患者的心功能,安全可靠,具有在临床中广泛推广的应用价值。Objective To analyze the clinical effect of torasemide and dopamine in the treatment of refractory heart failure in the elderly.Methods A total of 88 elderly patients with refractory heart failure were divided into experimental group and control group according to lottery method,with 44 cases in each group.The control group received dopamine therapy,and the experimental group received dopamine combined with torasemide therapy.Both groups were compared in terms of cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(ESV),left ventricular end-diastolic volume(EDV)]before and after treatment,therapeutic effects and occurrence of adverse reactions after treatment.Results After treatment,the LVEF(41.5±2.6)%in the experimental group was significantly higher than(32.5±2.2)%in the control group,and the ESV(100.5±1.8)ml and EDV(134.5±2.3)ml were significantly lower than(122.5±2.1)and(164.5±1.3)ml in the control group.All the differences were statistically significant(P<0.05).The total effective rate of the experimental group was 95.45%,which was higher than 70.45%of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the experimental group was 4.55%,which was lower than 18.18%in the control group,and the difference was statistically significant(P<0.05).Conclusion The combination of dopamine and torasemide has a significant clinical effect in the treatment of refractory heart failure in the elderly,and can significantly improve the cardiac function of patients.It is safe and reliable,and has the value of being widely promoted in clinical practice.
