摘 要:目的分析本院呼吸内科与骨科细菌耐药率情况,为临床确定药物治疗方案提供重要依据。方法1000份微生物标本,按科室不同分为A组(呼吸内科)与B组(骨科),每组500份。均进行微生物标本检验,比较两组耐药菌株检出情况;分析耐药菌株对抗菌药物的耐药率。结果耐药菌株主要以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌以及金黄色葡萄球菌为主。B组耐药菌检出率42%高于A组的21%,差异有统计学意义(P<0.05)。大肠埃希菌(107株)对氨苄西林的耐药率为80%(86/107);肺炎克雷伯菌(100株)对头孢唑啉的耐药率为42%(42/100);铜绿假单胞菌(60株)对氨曲南的耐药率为18%(11/60);金黄色葡萄球菌(50株)对青霉素G的耐药率为94%(47/50)。结论在本院临床微生物检验中监测细菌耐药率,能够了解呼吸内科与骨科耐药菌株分布情况,掌握不同耐药菌株对不同药物的耐药情况,指导临床合理使用抗菌药物,减少抗菌药物乱用情况的发生,有助于临床治疗工作开展。Objective To analyze the rate of bacterial resistance in respiratory medicine and orthopedics in our hospital,so as to provide an important basis for determination of treatment regimen.Methods 1000 microbial specimens were divided into group A(respiratory medicine)and group B(orthopedics)according to different departments,with 500 specimens in each group.All samples were tested for microorganisms,and the detection of drug-resistant strains in the two groups was compared;the resistance rate of drug-resistant strains to antibiotics was analyzed.Results The drug-resistant strains were mainly Escherichia coli,Klebsiella pneumoniae,Pseudomonas aeruginosa and Staphylococcus aureus.The detection rate of drug-resistant strains in group B was 42%,which was higher than 21%in group A,and the difference was statistically significant(P<0.05).The resistance rate of Escherichia coli(107 strains)to ampicillin was 80%(86/107);the resistance rate of Klebsiella pneumoniae(100 strains)to cefazolin was 42%(42/100);the resistance rate of Pseudomonas aeruginosa(60 strains)to aztreonam was 18%(11/60);the resistance rate of Staphylococcus aureus(50 strains)to penicillin G was 94%(47/50).Conclusion The monitoring of rate of bacterial resistance in clinical microbiology examination can understand the distribution of drug-resistant strains in respiratory medicine and orthopedics,grasp the resistance of different drug-resistant strains to different drugs,guide clinical rational use of antibacterial drugs,reduce the occurrence of misuse of antibiotics,which is help for clinical treatment.
