摘  要:目的分析四维超声时间空间相关成像(STIC)和超声断层显像技术(TUI)在胎儿先天性心脏病(FCHD)筛查中的应用价值。方法回顾性分析2017年5月至2021年5月在驻马店市第一人民医院进行孕期检查的存在高危FCHD风险的102例孕妇,均行二维超声和STIC-TUI检查,以引产或产后结果为"金标准",比较二维超声和STIC-TUI的诊断结果、诊断效能、检查时间及孕妇满意度。结果本组102例高危FCHD胎儿,经引产或产后证实FCHD 31例,正常71例;采用二维超声检查出FCHD 23例,正常60例;采用STIC-TUI诊断出FCHD 29例,正常69例。TIC-TUI诊断灵敏度、特异度、准确度分别为93.55%(29/31)、97.18%(69/71)、96.08%(98/102),高于二维超声诊断的74.19%(23/31)、84.51%(60/71)、81.37%(83/102);TIC-TUI漏诊率为6.45%(2/31),误诊率为2.82%(2/71),低于二维超声的25.81%(8/31)、15.49%(11/71),P<0.05。STIC-TUI样本耗时、每幅图像所用时间均短于二维超声,孕妇满意度高于二维超声(P<0.05)。结论STIC-TUI应用于FCHD筛查中,可提高诊断效果,避免漏诊,能降低缺陷儿出生率,确保新生人口质量。Objective To analyze the application value of four-dimensional ultrasound spatiotemporal image correlation(STIC)and tomographic ultrasound imaging(TUI)in screening fetal congenital heart disease(FCHD).Methods A total of 102 pregnant women with high risk of FCHD who underwent prenatal examination in Zhumadian First People’s Hospital from May 2017 to May 2021 were analyzed retrospectively.All patients underwent two-dimensional ultrasound and STIC-TUI.Taking the induced labor or postpartum results as the gold standard,the diagnostic results,diagnostic efficacy,examination time and pregnant women’s satisfaction to two-dimensional ultrasound and STIC-TUI were compared.Results Among the 102 high-risk fetuses with FCHD,31 cases were confirmed as FCHD,and 71 cases were normal after induced labor or after delivery;23 cases of FCHD and 60 normal cases were confirmed by two-dimensional ultrasound;29 cases of FCHD and 69 normal cases were confirmed by STIC-TUI.The diagnostic sensitivity,specificity and accuracy of STIC-TUI were 93.55%(29/31),97.18%(69/71),96.08%(98/102),respectively,which were higher than 74.19%(23/31),84.51%(60/71),81.37%(83/102)of two-dimensional ultrasound.The missed diagnosis rate of STIC-TUI was 6.45%(2/31),and the misdiagnosis rate was 2.82%(2/71),which were lower than 25.81%(8/31)and 15.49%(11/71)of two-dimensional ultrasound(P<0.05).Compared with two-dimensional ultrasound,STIC-TUI had shorter time consuming of sample and time of each images,but had higher satisfaction from pregnant women(P<0.05).Conclusions The application of STIC-TUI in FCHD screening can improve the diagnostic effect,avoid missed diagnosis,reduce the birth rate of defective children,and ensure the quality of neonatal population.
