摘　　要：目的:通过介绍临床药师炎症性肠病多学科(IBD-MDT)会诊的药学实践,为临床药师开展IBD-MDT药学服务提供参考。方法:介绍海南省人民医院IBD-MDT会诊的概况和工作流程;结合实践案例,总结分析IBD-MDT会诊的药学服务内容和工作模式。结果:临床药师可通过药物重整、治疗药物监测、药物治疗方案评估与制订、依从性评估及用药教育等方面发挥药学专业优势,参与IBD患者的诊治。结论:通过IBD-MDT会诊,临床药师可与临床形成专业合作、优势互补,提高医疗服务质量,优化慢病管理。OBJECTIVE To introduce the practice of clinical pharmacists multidisciplinary consultation on inflammatory bowel disease(IBD-MDT),so as to provide reference for clinical pharmacists to carry out IBD-MDT pharmaceutical care.METHODS The general situation and working process of IBD-MDT consultation in Hainan General Hospital were introduced.Combined with practical cases,the contents and working mode of pharmaceutical care in IBD-MDT consultation were summarized and analyzed.RESULTS Through medication reconciliation,therapeutic drug monitoring,medication regimens evaluation and formulation,compliance evaluation,medication education,clinical pharmacists could give full play to pharmaceutical advantages and participate in the diagnosis and treatment of IBD patients.CONCLUSION Through IBD-MDT consultation,clinical pharmacists can form professional cooperation with other clinical professionals,complement each other’s advantages,improve medical service quality and optimize chronic disease management.
