空心钉钛缆张力带内固定与克氏针钢丝张力带内固定对C型髌骨骨折的近远期疗效分析

摘　　要：对比空心钉钛缆张力带内固定与克氏针钢丝张力带内固定对C型髌骨骨折的近远期疗效.方法:于2018年2月-2020年3月选择在本院进行手术治疗的C型髌骨骨折患者110例作为研究对象,根据随机信封抽签原则把患者分为观察组与对照组各55例.观察组给予空心钉钛缆张力带内固定治疗,对照组给予克氏针钢丝张力带内固定治疗,记录与随访2组的近远期疗效.结果:所有患者都顺利完成手术,2组手术时间、术中出血量对比差异无统计学意义(P>0.05);观察组的术后住院时间与术后骨折愈合时间少于对照组(P<0.05).2组术后7天的血清IGF-1、Chemerin含量低于术前(P<0.05),观察组低于对照组(P<0.05).观察组术后6个月的疗效优良率为96.4%,高于对照组的81.8%(P<0.05).观察组随访1年的内固定松动、软组织炎症、关节粘连、切口感染等并发症发生率为3.6%,低于对照组的21.8%(P<0.05).结论:相对于克氏针钢丝张力带,空心钉钛缆张力带内固定治疗C型髌骨骨折能抑制IGF-1、Chemerin的表达,促进患者近期康复,也能提高患者的远期疗效优良率,降低并发症的发生.To compare the short-term and long-term effects of cannulated screw titanium cable ten-sion band internal fixation and Kirschner wire tension band internal fixation in the treatment of C-type patella frac-tures.Methods:From February 2018 to March 2020,110 cases of patients with type C patella fractures who under-went surgical treatment in our hospital were selected as the research subjects.Accorded to the principle of random en-velope drawing,the patients were divided into cannulated nail group and Kirschner wire group with 55 cases each groups.The cannulated screw group was given cannulated screw titanium cable tension band internal fixation,and the Kirschner wire group was given Kirschner wire tension band internal fixation.The short-term and long-term curative effects of the two groups were recorded and followed up.Results:All patients completed the operation suc-cessfully,and there was no significant difference in the operation time and intraoperative blood loss between the two groups(P>0.05).The postoperative hospital stay and postoperative fracture healing time of the cannulated screw group were shorter than those of the control group(P<0.05).The levels of serum IGF-1 and Chemerin in the two groups at 7 days after operation were lower than those before operation(P<0.05),and those in the cannulated screw group were lower than those in the K-wire group(P<0.05).The excellent and good rate of the cannulated screw group at 6 months after operation was 96.4%,which was higher than that of the Kirschner wire group(81.8%)(P<0.05).In the cannulated screw group,the 1-year follow-up rate of internal fixation loosening,soft tissue inflammation,joint adhesion,incision infection and other complications was 3.6%,which was lower than 21.8%in the control group(P<0.05).Conclusion:Compared with Kirschner wire tension band,cannulated screw and titanium cable tension band can inhibit the expression of IGF-1 and Chemerin in the treatment of C-type patella fracture,promote the short-term recovery of patients,and also improve the l
