从浊毒理论角度分析真菌性食管炎治疗新思路

摘　　要：真菌性食管炎是由于机体免疫力低下或免疫力缺陷进而感染真菌所导致的一种疾病。毛宇湘教授发现真菌的致病特点、临床表现与中医的浊毒致病理论相符,可将真菌归属于中医学的浊毒范畴。机体的免疫力与中医学中正气关系密切,正气不足导致机体免疫力下降,易产生浊毒,而脾胃虚弱又易导致机体正气不足,如此浊毒内蕴致真菌内生。因此,脾胃虚弱、浊毒内蕴为真菌性食管炎的基本病机,脾胃虚弱为本,浊毒内蕴为标。健脾胃与化浊毒法因疾病发展不同时期而有所侧重,临床治疗之时需审时度势,采用和胃8号方加减治疗,疗效确切。Fungal esophagitis is a disease caused by infection with mold due to low immunity or immune deficiency.Professor MAO Yuxiang found that the pathogenic characteristics and clinical manifestations of mold were consistent with the pathogenic theory of Zhuodu in traditional Chinese medicine.He believes that mold belongs to Zhuodu theory in traditional Chinese medicine.Immunity is closely related to the healty Qi in traditional Chinese medicine,the deficiency of healty Qi leads to the decline of human immunity and is easy to produce Zhuodu.The weakness of the spleen and stomach leads to the deficiency of healty Qi,and the zhuodu leads to the emergence of mold.Therefore,weakness of the spleen and stomach,Zhuodu accumulation is the basic pathogenesis of fungal esophagitis,weakness of the spleen and stomach is the root,and Zhuodu is the treetop.Invigorating the spleen and stomach and resolving Zhuodu have different emphasis due to different stages of the disease,clinical treatment needs to assess the situation and adopt the form of addition and subtraction of the main formula.The treatment with Hewei No.8 Formula has definite curative effect.
