逍遥散加减联合注射用鼠神经生长因子治疗原发性开角型青光眼的临床观察

摘　　要：目的观察逍遥散加减联合鼠神经生长因子对原发性开角型青光眼(primary open-angle glaucoma,POAG)患者的治疗效果。方法将我院POAG患者56例(100只眼)随机分为对照组和观察组,各28例(50只眼)。观察组采用逍遥散加减联合鼠神经生长因子治疗,对照组单用注射用鼠神经生长因子治疗,两组均治疗8周。观察两组治疗前后视力、眼压、视野平均敏感度(value and mean sensitivity,MS)、视野平均缺损(mean deviation,MD)、图像视觉诱发电位(pattern visual evoked potential,P-VEP)、神经纤维层厚度变化。结果治疗后,观察组视力提升、MS值增加、MD值减小,差异均有统计学意义(P<0.05);与对照组相比,观察组视力提升、MS值增加、MD值减小,差异均有统计学意义(P<0.05);两组治疗前后P-VEP、眼压、视盘和周边视网膜神经纤维层厚度比较,差异无统计学意义(P>0.05)。观察组总有效率高于对照组,差异有统计学意义(P<0.05)。结论对于POAG患者,逍遥散加减联合注射用鼠神经生长因子能有效改善患者视力下降、视野缺损的状态,是中西医结合治疗POAG的临床有效途径,值得临床推广应用。Objective To observe the therapeutic effect of modified Xiaoyao Powder combined with mouse nerve growth factor on patients with primary open-angle glaucoma(POAG).Methods A total of 56 POAG patients(100 eyes)in our hospital were randomly divided into control group and observation group,with 28 cases(50 eyes)in each group.The observation group was treated with modified Xiaoyao Powder combined with mouse nerve growth factor,and the control group was treated with mouse nerve growth factor for injection alone.Both groups were treated for 8 weeks.The visual acuity,intraocular pressure,value and mean sensitivity(MS),mean deviation(MD),pattern visual evoked potential(P-VEP),and changes in nerve fiber layer thickness of the two groups before and after treatment were observed.Results After treatment,the visual acuity of the observation group improved,the MS value increased,and the MD value decreased,and the differences were statistically significant(P<0.05).Compared with the control group,the observation group had improved visual acuity,increased MS value,and decreased MD value,the differences were statistically significant(P<0.05);there was no significant difference in P-VEP,intraocular pressure,optic disc and peripheral retinal nerve fiber layer thickness between the two groups before and after treatment(P>0.05).The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion For POAG patients,modified Xiaoyao Powder combined with mouse nerve growth factor for injection can effectively improve the state of vision loss and visual field defect in patients,it is a clinically effective way to treat POAG with integrated traditional Chinese and western medicine,and it is worthy of clinical application.
