摘 要:目的探讨能谱CT优化前置与后置ASiR-V技术在腹部CT增强扫描改善图像质量和降低辐射剂量的可行性研究。方法收集行上腹部能谱增强扫描患者50例为研究对象,动脉期(A组)、门脉期(B组)及延迟期(C组)分别采用前置30%ASiR-V、40%ASiR-V、50%ASiR-V,后置ASiR-V分别配对于和前置ASiR-V相同的权重,然后对延迟期再重建不同能谱的后置ASiR-V,分别为0%ASiR-V、30%ASiR-V、60%ASiR-V、90%ASiR-V。比较A、B、C 3组图像的图像质量及辐射剂量(CTDIvol、DLP、ED),分析不同权重前置ASiR-V对图像质量和辐射剂量的影响。结果不同前置ASiR-V A、B、C 3组图像的CTDLvol、DLP、ED比较,差异有统计学意义(P<0.05);C组与A组对比,CTDLvol、DLP、ED分别降低38%、38%、38%,与B组对比分别降低21%;A、B、C3组图像噪声竖脊肌SD、脂肪SD组间对比,差异无统计学意义(P>0.05)。比较延迟期再重建不同的后置ASiR-V的图像主观评分比较,差异均有统计学意义(P<0.05),结合主观评分ASiR-V60%评分最高。结论能谱CT采用迭代重建前置ASiR-V50%联合后置ASiR-V60%的能谱扫描和重建方案,可以显著降低能谱扫描的辐射剂量,同时能提高图像质量,在腹部能谱增强检查中具有重要的应用价值。Objective To explore the feasibility of improving image quality and reducing radiation dose in abdominal CT enhanced scanning with pre-ASiR-V and post-ASiR-V technology.Methods A total of 50 patients with the energy spectrum enhanced scanning of the upper abdomen were collected.The arterial phase(group A),the portal venous phase(group B)and the delayed phase(group C)were performed with 30%ASiR-V,40%ASiR-V and 50%ASiR-V respectively,and the post-ASiR-V was matched with the same weight as the pre-ASiR-V.Then,the post-ASiR-V with different energy spectra was reconstructed for the delay phase,which were 0%ASiR-V,30%ASiR-V,60%ASiR-V,and 90%ASiR-V,respectively.The image quality and radiation dose(CTDIvol,DLP,ED)of images in groups A,B,and C were compared,and the effects of pre-ASiR-V with different weights on image quality and radiation dose were analyzed.Results The differences of CTDLvol,DLP and ED in groupA,B and C with different pre-ASiR-V.were statistically significant(P<0.05).Compared with group A,CTDLvol,DLP and ED in group C were reduced by 38%,38%and 38%,respectively.Compared with group B,they were reduced by 22%,21%and 21%,respectively.There was no statistically significant difference in the image noise erector spinae SD and fat SD among the three groups(P>0.05).The subjective scores of different post-ASiR-V reconstruction images in delayed phase were compared,and the differences were statistically significant(P<0.05).Combined with the subjective scores,ASiR-V60%scored the highest.Conclusion Energy spectral CT adopts the iterative reconstruction pre-ASiR-V50%combined with post-ASiR-V60%energy spectral scanning and reconstruction scheme,which can significantly reduce the radiation dose of energy spectral scanning and improve the image quality at the same time.It has important application value in abdominal enhancement examination.
