贺州市暗娼人群艾滋病哨点监测及知晓率分析

摘　　要：目的 了解贺州市暗娼人群人类免疫缺陷病毒(human immunodeficiency virus, HIV)、梅毒螺旋体(梅毒)、丙型肝炎病毒(丙肝病毒)的感染情况和艾滋病(acquired immune deficiency syndrome, AIDS)知识知晓率变化趋势,为暗娼人群管理和防治措施的制定提供依据。方法 整理和分析2016—2020年贺州市暗娼人群AIDS哨点问卷和检测结果,利用SPSS软件进行统计学分析。结果 2016—2020年共监测贺州市暗娼人员3 572人,年龄为(36.85±9.29)岁,以40岁以下为主,占61.14%;文化程度以初中及以下水平为主,占94.26%;场所档次以中、低档为主,占92.00%。血清学结果与AIDS知晓率动态分析显示,5年间该人群HIV阳性率为1.51%,检出HIV阳性的暗娼中有53.70%是既往阳性;梅毒的阳性率为11.70%,逐年下降,差异有统计学意义;丙肝病毒的阳性率为1.12%;AIDS知晓率为92.19%,逐年增加,且AIDS知晓率和梅毒阳性率存在相关关系,呈负相关,随着AIDS知晓率升高,梅毒阳性率下降。多因素分析显示,暗娼人群AIDS知识知晓率与年龄、民族、文化程度、场所档次、在本地工作时间、接受干预情况、HIV和梅毒检测结果有关。结论 2016—2020年贺州市暗娼人群中HIV和梅毒阳性率较高,阳性暗娼继续从事卖淫活动较突出,需引起高度关注;暗娼人群学历普遍偏低,但AIDS知晓率逐渐提高,HIV和梅毒阳性检出率逐年下降,说明贺州市近年对暗娼人群开展的干预和治理措施积极有效。Objective To understand the prevalence of HIV, syphilis and hepatitis C infection and the awareness of HIV/AIDS among unlicensed prostitutes in Hezhou. Methods The AIDS sentinel questionnaire and testing results of unlicensed prostitutes in Hezhou city from 2016 to 2020 were obtained and were analyzed statistically using SPSS software. Results A total of 3 572 unlicensed prostitutes in Hezhou city from 2016 to 2020 were recruited, with the medan age of(36.85±9.29) years and 61.14% younger than 40 years old.94.26% were poorly educated and 92.00% offered their sex services at low level places.Among them, the overall HIV positive rate was 1.51% over the five-year period, and 53.70% were previously positive.The syphilis positive rate was 11.70%, which decreased significantly.The hepatitis C positive rate was 1.12%.The AIDS awareness rate was 92.19%, which increased significantly from 2016 to 2020.There was a negative linear correlation between AIDS awareness rate and syphilis positive rate.Multi-factor analysis showed that the HIV awareness rate was closely related to age, ethnicity, education level, level of service place, service years in local area, intervention acceptance, and HIV and syphilis test results. Conclusions HIV and syphilis positive rates are higher among unlicensed prostitutes in Hezhou city in 2016-2020.The unlicensed prostitutes who were positive for STD continue to engage in prostitution, which greatly increases the risk of transmission of STD.Most unlicensed prostitutes are poorly educated, however the AIDS awareness rate is gradually increasing.HIV and syphilis positive detection rates show down-ward trends, indicating that that the intervention and nagement measures carried out in recent years in Hezhou city for the unlicensed prostitute population are mapositive and effective.
