基于安德森卫生服务利用模型的居民就医选择现状及影响因素研究

摘　　要：目的以“北京市石景山区居民就医选择的行为现状”数据为依托,基于安德森卫生服务利用模型探索石景山区居民就医行为影响因素,识别关键影响因素,为医疗机构制定政策提供数据支撑。方法利用问卷调查法,于2021年9月对北京市石景山区居民进行抽样调查并获取数据。运用单因素分析、交叉分析等统计方法,了解居民就医行为现状,分析居民就医行为的影响因素。结果59.11%的居民初次就诊时会选择就近就医;居民区域内和区域外就医需求因素中居民健康状况自评项与使能因素中的13项比较均具有统计学意义(均P<0.01)。结论城市居民初次就诊时选择就近医院的比例大,使能因素、需要因素都对石景山区居民的就医行为有重要影响,且使能因素影响更为显著。在医疗管理上,医疗现状和水平需结合实际进行优化管理。Objective Based on the data of Research of residents'medical choice Shijingshan District,this paper aims to ex-plore the influencing factors of the residents'medical behaviors in Shijingshan District using the Anderson Health Service U-tilization Model,which will provide data support for the local medical institutions to formulate policies.Methods Using the method of questionnaire,a survey was conducted to obtain sample data of the residents in Shijingshan District in Sep-tember 2021.Single factor analysis,cross analysis and other statistical methods were implemented to analyze the medical willingness and influencing factors of residents in Shijingshan District.Results 59.11%residents choose their initial medi-cal treatment in Shijingshan District.The demand factors(P<0.01)of residents'health status self-assessment and enabling factors(P<0.01)are statistically significant.Conclusion The proportion of residents in Shijingshan District choosing their initial local medical treatment is large.Both enabling factors and demand factors have an important impact on the medical behavior of residents,and the impact of enabling factors is more significant.The medical status and level of Shijingshan District need to be optimized in combination with the local actual situation.
