摘  要:目的分析长期居住在不同空气污染程度地区儿童的肺功能差异,探讨适应当地的儿童肺功能参考值预测模型,为评价当地儿童健康水平提供科学依据。方法选择吉林省长春市和延吉市为研究城市,在每座城市的主城区分别选择2所小学,于2015—2019年每年的9—12月份,从每所学校的3~5年级小学生中每个年级随机抽取50人,共抽取3094名小学生作为研究对象,开展肺功能检测。肺功能检测指标包括FVC、FEV1、PEF、FEF25%和FEF75%。采用多重线性回归的方法建立各肺功能指标的参考值预测模型。结果除O_(3)外,长春市的PM2.5、PM10、SO_(2)、NO_(2)、CO等指标质量浓度均显著高于延吉市,差异有统计学意义(P<0.05);两城市儿童肺功能各项指标长春市普遍低于延吉市(Z分别=-22.454、-32.077、-17.945、-47.327、-17.732,均P<0.05),男生普遍高于女生(P<0.05);在不同地区,年龄、身高、体重对各项肺功能指标有不同程度的影响,差异均有统计学意义(均P<0.01)。结论长春市空气污染状况较延吉市严重。长期生活居住在空气污染物浓度较高地区的儿童,其肺功能较差,生长发育指标中身高对肺功能各项指标的影响最大也最广泛。Objective To analyze the differences of lung function of children living in different air pollution areas for a long time,and a prediction model of children's lung function reference value adapted to the local area was established to provide a scientific basis for evaluating the health level of local children.Methods Two cities of Changchun and Yanji in Jilin Province were selected as the study cities,and two primary schools were selected in the main urban area of each city.From September to December every year from 2015 to 2019,50 students were randomly selected from each Grade 3 to Grade 5 in each school.A total of 3094 pupils were selected for lung function test.Pulmonary function tests included FVC,FEV1,PEF,FEF25%and FEF75%.The multiple linear regression was used to establish the reference value prediction model of each pulmonary function index.Results The air pollution in Changchun was more serious than that in Yanji.Except for O_(3),the mass concentrations of PM2.5,PM10,SO_(2),NO_(2)and CO in Changchun were significantly higher than those in Yanji(P<0.05).Age,height and weight had different effects on pulmonary function indexes in different areas(P<0.05).The fitting effect of girls was better than that of boys,Yanji was better than Changchun.Conclusion Children who live in areas with high concentration of air pollutants for a long time have poor lung function.Height had the greatest and most ex-tensive influence on various indexes of lung function in the indicators of growth and development.
