右美托咪定联合罗哌卡因肌间沟神经阻滞对患者镇痛效果及脑电双频指数值的影响

摘　　要：目的探讨右美托咪定联合罗哌卡因肌间沟神经阻滞对患者镇痛效果及脑电双频指数(BIS)值的影响。方法选择2019年4月至2021年3月北京市怀柔区中医医院收治的83例拟行上臂手术患者作为研究对象,采用随机数字表法将其分为观察组(41例)和对照组(42例)。对照组患者给予罗哌卡因联合安慰剂阻滞,观察组患者采用罗哌卡因联合右美托咪定阻滞。比较两组患者的镇痛起效时间、镇痛持续时间、运动阻滞起效时间及运动恢复时间,同时比较两组术中不同时刻的BIS值变化情况。结果两组患者的镇痛起效时间比较,差异无统计学意义(P>0.05);观察组患者的镇痛持续时间长于对照组,差异有统计学意义(P<0.05);两组患者的运动阻滞起效时间比较,差异无统计学意义(P>0.05);观察组患者的运动恢复时间长于对照组,差异有统计学意义(P<0.05)。观察组患者T_(1)~T_(5)的BIS值均低于对照组,差异有统计学意义(P<0.05)。观察组患者T_(1)时刻的BIS值低于T_(0),T_(2)时刻的BIS值低于T_(1),T_(4)时刻的BIS值高于T_(3),差异有统计学意义(P<0.05)。对照组患者T_(2)时刻的BIS值低于T_(1),T_(3)时刻的BIS值低于T_(2),T_(4)时刻的BIS值高于T_(3),T_(5)时刻的BIS值高于T_(4),差异有统计学意义(P<0.05)。结论右美托咪定联合罗哌卡因肌间沟神经阻滞,可延长患者运动恢复时间及镇痛持续时间,提高患者麻醉效果。Objective To investigate the effect of Dexmedetomidine combined with Ropivacaine intermuscular sulcus nerve block on the analgesic effect and bispectral index(BIS)value of patients.Methods A total of 83 patients undergoing upper arm surgery admitted to Huairou District Hospital of Traditional Chinese Medicine in Beijing from April 2019 to March 2021 were selected as the research objects,and they were divided into observation group(41 cases)and control group(42 cases)by random number table method.Control group was given Ropivacaine combined with placebo block,and observation group was given Ropivacaine combined with Dexmedetomidine block.The onset time of analgesia,duration of analgesia,onset time of exercise block and recovery time of exercise were compared between the two groups,and the changes of BIS values at different intraoperative time were compared between the two groups.Results There was no significant difference in the onset time of analgesia between the two groups(P>0.05).The duration of analgesia in observation group was longer than that in control group,and the difference was statistically significant(P<0.05).There was no significant difference in the effective time of movement block between the two groups(P>0.05).The exercise recovery time of observation group was longer than that of control group,and the difference was statistically significant(P<0.05).BIS values of T_(1)-T_(5) in observation group were lower than those in control group,the differences were statistically significant(P<0.05).In the observation group,BIS value at T_(1) was lower than T_(0),BIS value at T_(2) was lower than T_(1),and BIS value at T_(4) was higher than T_(3),the differences were statistically significant(P<0.05).In the control group,BIS value at T_(2) was lower than T_(1),BIS value at T_(3) was lower than T_(2),BIS value at T_(4) was higher than T_(3),and BIS value at T_(5) was higher than T_(4),the differences were statistically significant(P<0.05).Conclusion Dexmedetomidine combined with Ropivacaine intermuscular sulc
