医学院校新生自杀意念潜在类别分析研究

摘　　要：目的:了解医学院校新生当下、既往自杀意念的潜在类别最佳个数、形态、构成比及自杀意念潜在类别因子间关系,为做好大学生心理健康工作提供依据。方法:对2018、2019年入学的哈尔滨医科大学大庆校区新生人群进行横断面研究,共调查4249人,调查其当下、既往自杀意念情况。结果:大学新生当下、既往的自杀意念最佳潜在类别分类分别为3类和4类,形态略有差异。4249名大学新生中,当下无自杀意念组、中度自杀心理和行为计划组、高度自杀心理和行为计划组,构成比分别为90.8%、6.9%、2.3%;既往无自杀意念组、中度自杀心理和行为计划组、中度自杀心理和无行为计划组、高度自杀心理和行为计划组,构成比分别为76.3%、13.1%、5.5%、5.1%。当下与既往的自杀意念潜在类别因子是两个独立的因子,且有较高相关系数0.855,差异有统计学意义(P<0.01)。结论:医学院校新生当下和既往自杀意念阳性检出率均较高,但二者类别数不一致,隶属于不同的潜在类别因子,既往自杀意念可能是当下自杀意念的危险因素。虽然当下自杀意念阳性率要低于既往自杀意念阳性率,但只要出现自杀意念就包含自杀行为计划,极有可能会出现自杀行为。应积极关注医学院校新生自杀意念状况,在入学时便根据自杀意念类型特点做出积极干预。OBJECTIVE To study the best number,form and constituent ratio of latent class of current and past suicidal ideation in medical university freshmen,to explore the relationship between latent class factors of previous and current suicidal ideation.METHODS A cross-sectional study was conducted on the freshmen enrolled in Harbin Medical University(Daqing)in 2018 and 2019 to investigate the current and past suicidal ideation.RESULTS The best latent class of current and past suicidal ideation of col⁃lege freshmen are 3 and 4 respectively,with slight differences in morphology.The latent class factors of current and past suicidal ide⁃ation all had no-suicidal-ideation cluster,moderate-psychological and behavioral-planning cluster,high-psychological and behavior⁃al-planning cluster,and the constituent ratios were 0.908,0.069,0.023 and 0.763,0.131 and 0.051 respectively.The latent class factor of past suicidal ideation was also divided into moderate-psychological and unplanned-behavior cluster,its constituent ratio was 0.055.The class factors of present and past latent suicidal ideation were two independent factors,and the correlation coefficient was 0.855(P<0.001).CONCLUSION The positive detection rates of current and past suicidal ideation of medical university fresh⁃men were high,but the numbers of two latent class factors’clusters were inconsistent.Two latent class factors belonged to different potential category factors.Past suicidal ideation may be a risk factor of current suicidal ideation.Although the current positive rate of suicidal ideation was lower than the previous positive rate of suicidal ideation,if current suicidal ideation appeared,it must in⁃clude suicidal behavior-planning,Suicidal behavior may be highly.It is suggested that we should pay active attention to the situa⁃tion of suicidal ideation in medical university freshmen,make active intervention according to the characteristics of suicidal ideation at the time of enrollment.
