摘　　要：目的:了解武汉市居民对家庭医生签约服务的知晓及利用情况,为完善城市家庭医生签约服务提供政策建议。方法:2019年7月对武汉市4个社区749名≥16岁的常住居民进行问卷调查。结果:749名被调查居民中,签约居民252人,签约率为33.7%,其中主要为≥60岁的老年人(51.6%)和慢性病人(52.8%)。在签约居民中,80.6%知道家庭医生的姓名,47.2%看病时首选自己的家庭医生,88.1%对家庭医生签约服务总体满意,82.9%有续签意愿。多因素logistic回归分析结果显示,≥60岁、首选社区卫生机构、听说过家庭医生签约服务、了解家庭医生签约服务、认为家庭医生对解决健康问题有帮助是促进居民选择家庭医生签约服务的影响因素,差异有统计学意义(OR值分别为2.114、1.945、14.068、3.697和2.334,P值均<0.05)。结论:武汉市家庭医生签约服务处于较低水平,居民知晓和利用不足,需要相关部门强化宣传,使家庭医生签约服务紧扣居民需求,并向普通人群延伸。OBJECTIVE To understand the awareness,utilization,and satisfaction of family doctors’contract services among Wuhan residences and to provide policy recommendations for improving the family doctors’contract service system.METHODS Questionnaire survey was conducted among 749 residents aged 16 and above in 4 communities,Wuhan.RESULTS Among residents participated in the study,33.7%had signed a contract,mainly the elderly(51.6%)and chronic patients(52.8%).Among the contracted residents,80.6%knew the name of family doctors,47.2%saw their own family doctor at first visit,88.1%were generally satisfied with the family doctors’contract services,and 82.9%had the intention to renew.The logistic regression results show that age 60above,selecting community health service institutions for the first visit,heard of family doctor contract services,understanding family doctor’contract services,believing that family doctors contribute to solve health problems are the influencing factors of whether residents sign a contract.CONCLUSION The level of family doctors’contract services among Wuhan population is low,and residents’awareness and utilization of family doctors’contract services are insufficient.Contract services need to strengthen policy advocacy,closely follow the demands of residents,and extend to the general population.
