摘　　要：目的:评价药物涂层球囊治疗冠状动脉大血管原发病变的有效性和安全性。方法:连续收集2020年1~10月中国医学科学院阜外医院接受药物涂层球囊治疗冠状动脉大血管(直径≥3.0 mm)原发病变的137例患者,记录患者一般信息、病史、手术资料,以及随访12个月期间心原性死亡、心肌梗死、靶血管血运重建的发生情况。结果:137例患者共有143处冠状动脉大血管原发病变,13处(9.1%)为A型病变,92处(64.3%)为B型病变,38处(26.6%)为C型病变,血管平均参考直径(3.1±0.3)mm。住院期间无1例发生急性心肌梗死、心原性死亡、靶血管血运重建以及急性冠状动脉内血栓形成。1例因预扩张后严重夹层置入1枚支架。随访12个月期间无1例发生急性心肌梗死、心原性死亡、靶血管血运重建。结论:药物涂层球囊治疗冠状动脉大血管原发病变安全、有效,预后良好。Objectives:To evaluate the efficacy and safety of drug-coated balloon therapy for patients with de novo lesions of large coronary vessels.Methods:A total of 137 patients were treated with drug-coated balloon for de novo lesions of large coronary vessels.General information,medical history,and procedural information were obtained,and cardiac events(cardiogenic death,myocardial infarction,target vessel revascularization)during the 12-month follow-up period were recorded.Results:Among 143 de novo lesions in 137 patients,13(9.1%)lesions were classified as type A,92(64.3%)as type B,and 38(26.6%)as type C.The reference vessel diameter was(3.1±0.3)mm.No acute myocardial infarction,no cardiac death,no target vessel revascularization,and no acute coronary thrombosis occurred during hospitalization.One patient underwent bail-out stent implantation because of serious coronary dissection.During the 12-month follow-up period,no acute myocardial infarction,no cardiac death,and no target vessel revascularization occurred.Conclusions:Drug-coated balloon therapy is safe and effective for de novo lesions of large coronary vessels,and the prognosis of this procedure is satisfactory.
