摘　　要：目的:探讨成人Swyer-James-MacLeod综合征(SJMS)的多排螺旋CT(multi-detector spiral CT,MDCT)的诊断价值。方法:回顾分析2019年6月—2020年12月丰县中医医院收治的40例成人Swyer-James-MacLeod综合征的临床及MDCT资料,所有患者均行MDCT扫描及其多平面重建(multiplanar reconstruction,MPR)等后处理。结果:胸部MDCT显示累及局部肺段(n=21)、肺叶(n=14)或单侧全肺(n=5)的透亮度增加、肺野密度明显降低,支气管血管减少或变细,部分可伴肺门变小(n=7),患侧肺容积减少(n=9),纵隔向患侧轻微移位(n=9)等特征性表现。部分病例还可见支气管扩张(n=35)、肺结核(n=5)及肺动脉高压(n=4)等伴发症。结论:MDCT对成人SJMS的特征显示较佳,还可诊断伴发症,值得应用。Objective To investigate the diagnostic value of multi-detector spiral CT(MDCT)in adult Swyer-James-MacLeod syndrome(SJMS).Methods The clinical and MDCT data of 40 adults with Swyer-James-MacLeod syndrome treated in Fengxian traditional Chinese medicine hospital from June 2019 to December 2020 were retrospectively analyzed.All patients underwent MDCT scan,its multiplanar reconstruction(MPR)and other post-processing.Results SJMS may be identified by chest MDCT,showing hyperlucent or decreased density in the lung field of the affected lung segment(n=21),lung lobe(n=14)or whole unilateral lung(n=5)with reduced or narrowed bronchovascular markings,partly with a smaller hilus(n=7),and reduced lung volume(n=9)with slight displacement of the mediastinum to the affected side(n=9).Bronchiectasis(n=35),pulmonary tuberculosis(n=5),pulmonary hypertension(n=4)and other concomitant diseases were seen in some patients.Conclusion MDCT can better display the characteristics of SJMS in adults,and can also diagnose its concomitant diseases,which is worthy of application.
