摘  要:目的建立和评估浙江省健康人群肺功能预测方程。方法选择2018年6月至2020年6月在浙江省33家临床中心进行肺功能检查的健康体检者4264例,采用随机数字表法分为研究组2141例和验证组2123例,每组再分为男性亚组和女性亚组。年龄≥65岁的老年对象1168例参照前述方法进行分组,其中老年研究组568例,老年验证组600例。收集一般资料并作肺功能检测。研究组分不同性别,采用逐步多元回归分析建立肺功能预测方程,获得研究组和验证组肺功能参数预测值。计算肺功能参数预测值与实测值之间的比值,获得肺功能参数预测百分比。比较研究组与验证组肺功能参数预测百分比,以此验证肺功能预测方程的可行性。老年研究组和验证组亦采用上述方法建立和评估男女不同性别肺功能预测方程。结果两组对象的一般资料及肺功能参数比较,差异均无统计学意义(均P>0.05)。男性、女性用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC、呼气峰流量(PEF)、最大呼气中期流量(MMEF)、用力呼气25%肺活量的瞬间流量(FEF25%)、FEF50%、FEF75%、一氧化碳弥散量(DLCO)均与各自年龄呈负相关,男性最大分钟通气量(MVV)亦与年龄呈负相关。男性FVC、FEV_(1)、PEF、MMEF、FEF25%、FEF50%、FEF75%、MVV与身高呈正相关;女性FEV_(1)/FVC与身高呈负相关,其他参数均与身高呈正相关。男性PEF和DLCO与体重呈正相关,FEV_(1)/FVC和FEF75%与体重呈负相关;女性DLCO与体重呈正相关,FEV_(1)/FVC与体重呈负相关。老年男性、女性FVC、FEV_(1)、PEF、FEF25%、FEF50%、FEF75%等肺功能参数均与年龄呈负相关;老年男性、女性FVC、FEV_(1)、DLCO以及老年男性PEF、FEF25%,老年女性MMEF、FEF50%均与身高呈正相关。老年男性MMEF和老年女性DLCO与体重呈正相关,老年女性FEV_(1)、MMEF与体重呈负相关。研究组、验证组,老年研究组、老年验证组肺功能参数Objective To establish and validate a prediction equations of lung function for healthy population in Zhejiang Province.Methods A total of 4264 people who underwent lung function examination in 33 clinical center in Zhejiang Province from June 2018 to June 2020 were recruited into the research and they were randomly divided into training cohort(n=2141)and validation cohort(n=2123).Each training and validation cohort were divided into male subgroup and female subgroup.There were 1168 cases aged≥65 years(elderly group),among whom 568 were randomly assigned in elderly training cohort and 600 in elderly validation cohort.The general information of subjects were documented;and the lung function was measured with spirometry.The stepwise multiple regression analysis was used to establish the prediction equation of lung function for male and female study subgroup,and the predicted values of lung function parameters were obtained from study group and validation group with new predictive equation.The ratio between the predicted value and the measured value of pulmonary function parameters was calculated to obtain the predicted percentage of pulmonary function parameters.The prediction percentage of lung function parameters in the study group and the validation group were used to verify the feasibility of the lung function prediction equation.The elderly study group was used to establish the male and female prediction equations and validation group was used to evaluate the feasibility with the previous method.Results There were no significant differences in general information and lung function parameters between training cohort and validation cohort(all P>0.05).The FVC,FEV_(1),FEV_(1)/FVC,PEF,MMEF,FEF25%,FEF50%,FEF75%and DLCO were negatively correlated with ages in both genders;the MVV was also negatively correlated with age in males.The FVC,FEV1,PEF,MMEF,FEF25%,FEF50%,FEF75%and MVV were positively correlated with the heights in males;the FEV_(1)/FVC was negatively correlated with height in females,while other parameters
