摘  要:目的探讨ApoE基因多态性与高血压患者血脂及尿酸代谢之间的相关性。方法选择2017年3月1日至2018年1月31日收住福建省立医院的高血压患者417例。排除ε2/ε4基因型后,将患者分为野生型组(ε3/ε3,274例)、携带ε2等位基因组(ε2/ε2+ε2/ε3,76例)以及携带ε4等位基因组(ε3/ε4+ε4/ε4,60例),比较3组血脂及尿酸代谢水平的差异。结果与野生型组相比,携带ε2等位基因组的总胆固醇[4.74(3.72,5.43)mmol/L]及低密度脂蛋白胆固醇[2.86(2.15,3.68)mmol/L]水平较低;而携带ε4等位基因组的总胆固醇[5.94(5.17,6.94)mmol/L]、低密度脂蛋白胆固醇[4.16(3.53,4.92)mmol/L]以及甘油三酯[3.26(1.90,4.90)mmol/L]水平均较高(P<0.016),同时高密度脂蛋白胆固醇[0.95(0.78,1.17)mmol/L]水平较低(P<0.0167)。进一步研究影响甘油三酯、总胆固醇及血尿酸升高的因素,发现若以野生型组为参照,则携带ε4等位基因既是甘油三酯水平升高的影响因素(OR=2.582,95%CI:1.245~5.352,P=0.011),也是总胆固醇水平升高的影响因素(OR=2.200,95%CI:1.121~4.315,P=0.022),而携带ε2等位基因是尿酸水平升高的影响因素(OR=1.859,95%CI:1.035~3.338,P=0.038)。结论在高血压患者中,ApoE基因多态性与血脂及尿酸代谢均存在相关性,与野生型相比较,携带ε4等位基因与总胆固醇及甘油三酯水平升高均有关,而携带ε2等位基因与尿酸水平升高有关。Objective To explore the correlation between genetic polymorphism of apolipoprotein E(ApoE)and lipid and serum uric acid metabolism in patients with hypertension.Method A total of 417 hypertensive patients were selected for this study from March 1,2017 to January 31,2018.The subjects were divided into the wild-type group(ε3/ε3,n=274),the group carrying theε2 allele(ε2/ε2+ε2/ε3,n=76),and the group carrying theε4 allele(ε3/ε4+ε4/ε4,n=60).We compared the differences in lipid and serum uric acid metabolism among these three groups.Result Compared with the wild-type group,the group carrying theε2 allele had lower total cholesterol[4.74(3.72,5.43)mmol/L vs 5.15(4.30,5.74)mmol/L]and low-density lipoprotein cholesterol[2.86(2.15,3.68)mmol/L vs 3.46(2.70,4.00)mmol/L],whereas the group carrying theε4 allele possessed higher total cholesterol[5.94(5.17,6.94)mmol/L vs 5.15(4.30,5.74)mmol/L],low-density lipoprotein cholesterol[4.16(3.53,4.92)mmol/L vs 3.46(2.70,4.00)mmol/L] and triglyceride [3.26(1.90, 4.90)mmol/L vs 1.72(1.13, 2.38)mmol/L] (P < 0.016). Compared with the wild-type group, the ε4 allele carriers had lower high-density lipoprotein cholesterol [0.95(0.78, 1.17)mmol/L vs 1.07(0.89, 1.30)mmol/L] (P < 0.016). Furthermore, setting the wild-type as reference, multivariate logistic regression analysis showed that ε4 allele positively corelated with hypertriglyceridemia (OR=2.582, 95%CI: 1.245- 5.352, P=0.011) and hypercholesterolemia (OR=2.200, 95%CI: 1.121- 4.315, P=0.022), while ε2 allele carriers and hyperuricemia were positively correlated (OR=1.859, 95%CI: 1.035- 3.338, P=0.038). Conclusion ApoE polymorphism is associated with serum lipids and uric acid metabolism in hypertensive patients. Compared with the wild-type, ε4 allele in hypertensive patients is related to elevated triglycerides and total cholesterol, while the ε2 allele is related with increased serum UA level.
