摘  要:目的探讨局部高流量氧疗联合综合性护理对放射性皮肤损伤的临床疗效及相关影响因素。方法选取2018年11月至2020年5月河北省沧州中西医结合医院收治的90例出现放射性皮肤损伤的头颈部肿瘤患者为观察组。另择河北省沧州中西医结合医院同期收治的90例未出现放射性皮肤损伤的头颈部肿瘤患者为对照组。采用随机数字表法将观察组患者分为A组(给予常规治疗)、B组(给予局部低流量氧疗联合综合性护理)和C组(给予局部高流量氧疗联合综合性护理),每组各30例。干预1个月后,比较分析A、B、C组患者的临床疗效,皮肤损伤程度和生存质量。采用多因素logistic回归方法分析患者发生放射性皮肤损伤的独立影响因素。结果干预1个月后,B组和C组患者的伤口渗液量情况、伤口气味、生存质量、创面愈合时间均显著优/短于A组(P<0.05),且C组显著优/短于B组(P<0.05);B组和C组患者的不良反应总发生率显著低于A组(P<0.05),且C组显著低于B组(P<0.05);C组患者的皮肤损伤程度显著低于A组和B组(P<0.05)。多因素logistic回归分析结果显示,氧流量是患者发生放射性皮肤损伤的独立影响因素(P<0.05)。结论氧流量是放射性皮肤损伤的独立保护因素,且采用局部高流量氧疗联合综合性护理措施可显著促进患者伤口愈合,改善皮肤损伤程度,降低不良反应发生率,提高患者的生存质量。Objective To explore the effects of local high flow oxygen therapy combined with comprehensive nursing to radiation skin injury and its influencing factors.Method 90 cases of head and neck tumor patients with radioactive skin injury treated in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine in Hebei Province from November 2018 to May 2020 were selected as the observation group.In addition,90 cases of head and neck tumor patients without radiation skin injury admitted to our hospital during the same period were selected as the control group.The observation group was divided into group A(routine treatment),group B(local low flow oxygen therapy combined with comprehensive nursing)and group C(local high flow oxygen therapy combined with comprehensive nursing)by random number table method,with 30 patients in each group.After 1 month of intervention,the clinical efficacy,skin injury degree and quality of life of patients in group A,group B and group C were compared and analyzed.To analyze the independent influencing factors of radiation skin injury in patients.Result After 1 month of intervention,the amount of wound exudation,wound odor,quality of life and wound healing time in group B and group C were significantly better and shorter than those in group A(P<0.05),and group C were significantly better and shorter than those in group B(P<0.05).The total incidence of adverse reactions in group B and group C was significantly lower than that in group A(P<0.05),and group C was significantly lower than that in group B(P<0.05).The degree of skin injury in group C was significantly lower than that in group A and group B(P<0.05).Multivariate logistic regression analysis showed that oxygen was independent influencing factors for radiation skin injury(P<0.05).Conclusion Oxygen flow is an independent protective factor of radioactive skin injury,and the use of local high flow oxygen therapy combined with comprehensive nursing can significantly promote the wound healing of patients,improve the skin injury s
