幽门螺杆菌感染肝硬化中血清降钙素原、血氨水平和白介素-8表达水平与预后的相关性

摘　　要：目的分析幽门螺杆菌(Hp)感染肝硬化血清降钙素原(PCT)、血氨水平、白介素-8(IL-8)表达水平,并探讨与预后相关性。方法以随机样本抽样法,于我院(2019年3月-2021年4月)收治的肝硬化患者中抽取104例,根据胃黏膜尿素酶/13C尿素呼气试验结果,分为未感染Hp组(n=54)、感染Hp组(n=50),对比两组血清PCT、血氨水平、IL-8水平进行,探讨与预后相关性。结果感染Hp组血清PCT、血氨水平、IL-8水平高于未感染Hp组,P<0.05;未感染Hp组Child分级中A级显著高于感染Hp组,C级占比显著低于感染Hp组(P<0.05);两组Child分级中B级占比比较,P>0.05;血清PCT、血氨、IL-8水平与Child分级均呈正相关关系(r=0.651/0.735/0.798,P<0.001)。结论Hp感染患者PCT、血氨、IL-8水平明显高于非Hp感染的患者,与预后Child分级呈正相关关系,对于后续治疗具有一定指导意义。Objective To analyze the levels of serum procalcitonin(PCT),blood ammonia and the expression of interleukin-8(IL-8)in patients with Helicobacter pylori(HP)infected cirrhosis,and to explore the correlation with prognosis.Methods By random sample sampling method,104 patients with liver cirrhosis were selected from our hospital(March 2019 April 2021).According to the results of gastric mucosal urease/13c urea breath test,they were divided into non infected HP group(n=54)and infected HP group(n=50).The levels of serum PCT,serum ammonia and IL-8 were compared between the two groups to explore the correlation with prognosis.Results The levels of serum PCT,serum ammonia and IL-8 in infected HP group were higher than those in uninfected HP group(P<0.05);The child grade A in non infected HP group was significantly higher than that in infected HP group,and the proportion of grade C was significantly lower than that in infected HP group(P<0.05);The proportion of grade B in child grading of the two groups was compared(P>0.05);The levels of serum PCT,serum ammonia and IL-8 were positively correlated with child grade(r=0.651/0.735/0.798,P<0.001).Conclusion The levels of PCT,blood ammonia and IL-8 in patients with HP infection are significantly higher than those in patients without HP infection,which is positively correlated with the child grade of prognosis,and has certain guiding significance for follow-up treatment.
