血β2微球蛋白与老年2型糖尿病患者尿蛋白的相关性研究

摘　　要：目的探讨血β2微球蛋白(β2M)与老年糖尿病患者尿蛋白的关系。方法选取2018年5月~2019年5月来首都医科大学宣武医院住院诊治的老年糖尿病患者165例,按照尿白蛋白肌酐比值(UACR)分为正常蛋白尿组(A组,100例)、微量蛋白尿组(B组,40例)和大量蛋白尿组(C组,25例)。收集患者的临床资料和生化指标,用Spearman秩相关和logistic回归分析UACR的危险因素。结果β2M、糖化血红蛋白(HbA1c)和血清肌酐(Cr)在C组高于A组和B组,且B组高于A组,差异有统计学意义(P<0.05)。Spearman分析显示UACR与β2M、HbA1c、Cr、尿素氮(BUN)呈正相关(P<0.05),行logistic回归分析显示β2M、HbA1c、Cr是微量蛋白尿独立危险因素。在3组中β2M的阳性检出率均高于Cr和肾小球滤过率(GFR)下降的阳性率。结论β2M是老年糖尿病患者微量蛋白尿的独立危险因素,在蛋白尿早期诊断中的价值高于Cr。Objective To explore the association between serumβ2-microglobulin(β2M)and albuminuria in elderly patients with type2 diabetes.Methods In this study 165 elderly diabetic patients cared at Xuanwu hospital from May 2018 to May 2019 were randomly selected.Patients were divided into three groups according to their urinary albumin-to-creatinine ratio(UACR):normal albuminuria group(A group=100 cases),micro-albuminuria group(B group=40 cases)and a large amount of albuminuria group(C group=25 cases).Clinical data and biochemical data were collected.Spearman rank-related and Logistic regression were used to analyze the risk factors of UACR.Results The level ofβ2M,as well as glycated hemoglobin(HbA1c)and serum creatinine(Cr),in the C group were higher than those in the B group and the A group.Meanwhile,those in the B group were higher than the A group,and the difference was statistically significant(P<0.017).UACR had positive correlation withβ2M,HbA1c,Cr and blood urea nitrogen(BUN)according to Spearman correlation analysis(P<0.05).Logistic regression analysis showed thatβ2M,HbA1c,and Cr were independent risk factors of micro-albuminuria.The positive detection rate ofβ2M in all three groups was higher than that of Cr and decreased glomerular filtration rate(GFR).Conclusionβ2M is an independent risk factor for micro-albuminuria in elderly patients with diabetes mellitus,and its value in early diagnosis of albuminuria is higher than that of Cr.
