摘 要:目的 探讨异氟烷麻醉对脊柱骨折大鼠神经功能、镇痛及脊髓c-FOS蛋白的影响。方法 随机将40只大鼠分为4组,健康组(Health group, HEA组)、模型组(Model group, MOD组)、低剂量异氟烷组(Low-dose isoflurane group, LDI组)、高剂量异氟烷组(High-dose isoflurane group, HDI组)。酶联免疫法检测大鼠血清中炎性因子水平,参照Longa及Bederson对大鼠神经功能进行评分,Western blot法及免疫组化法检测大鼠脊髓背角组织中c-FOS及SP蛋白表达。结果 与HEA组比较,MOD组、LDI组和HDI组大鼠神经功能评分指数、各时间段足伸姿推力(Elevated posture thrust, EPT)、TNF-α及IL-6水平、c-FOS及SP蛋白表达及阳性表达率升高,各时间段后肢撤足阈值(Paw withdrawal threshold, PWT)均降低(P<0.05);与MOD组相比,LDI组和HDI组大鼠神经功能评分指数降低,大鼠神经功能评分指数、各时间段EPT、TNF-α、IL-6水平、c-FOS及SP蛋白表达以及阳性表达率降低,各时间段PWT均升高(P<0.05);与LDI组相比,HDI组大鼠神经功能评分、各时间段EPT、TNF-α、IL-6水平、c-FOS及SP蛋白表达以及阳性表达率降低更明显,各时间段PWT均升高更明显(P<0.05)。结论 异氟烷可降低脊柱骨折大鼠疼痛效应,提高神经功能,抑制炎性因子表达,其作用机制可能与抑制脊髓背角组织中c-FOS及SP蛋白表达有关。Objective To explore the effect of isoflurane anesthesia on nerve function,analgesia and spinal cord c-FOS protein in rats with spinal fracture.Methods Totally 40 rats were randomly divided into 4 groups,health group(HEA group),model group(MOD group),low-dose isoflurane group(LDI group),and high-dose isoflurane group(HDI group).Enzyme-linked immunoassay was used to detect the level of inflammatory factors in rat serum;refering to Longa and Bederson,the neurological function of rats was scored;Western blot and immunohistochemical methods were used to detect the expression of c-FOS and SP protein in the spinal dorsal horn tissue of rats.Results Compared with HEA group,the neurological function score index,EPT,TNF-αand IL-6 levels,c-FOS and SP protein expression and the positive expression rate increased,while PWT decreased in each time period(P<0.05)in MOD,LDI and HDI groups.Compared with MOD group,the rats′neurological function score index of LDI group and HDI group decreased,and the rat neurological function score index,EPT,TNF-αand IL-6 levels,c-FOS and SP protein expression and the positive expression rate decreased,while PWT increased in each time period(P<0.05).Compared with LDI group,the neurological function score of HDI group and the levels of EPT,TNF-αand IL-6,c-FOS and SP protein expression and the positive expression rate decreased significantly,while PWT increased more significantly in each time period(P<0.05).Conclusion Isoflurane can reduce the pain effect of spinal fracture rats,improve the nerve function,and inhibit the expression of inflammatory factors.Its mechanism may be related to the inhibition of c-FOS and SP protein expression in the spinal dorsal horn tissue.
