单硝酸异山梨酯缓释片联合增强型体外反搏对UAP的疗效

摘　　要：目的:研究单硝酸异山梨酯缓释片联合增强型体外反搏(EECP)治疗不稳定型心绞痛(UAP)的疗效。方法:于我院治疗的112例UAP被随机均分为药物治疗组(常规治疗基础上加用单硝酸异山梨酯缓释片)与联合治疗组(药物治疗组基础上加用EECP),两组均治疗28d。比较两组治疗前后西雅图心绞痛调查表(SAQ)评分、ST段压低程度、血浆血管假性血友病因子(vWF)、纤维结合蛋白(Fn)水平、血清肿瘤坏死因子(TNF)-α、单核细胞趋化蛋白(MCP)-1水平及治疗总有效率。结果:与药物治疗组比较,治疗后,联合治疗组SAQ评分[(74.94±10.22)分比(80.33±9.90)分]升高更显著,ST段压低程度[(0.23±0.12)mV/周平均比(0.11±0.06)mV/周平均]、血浆vWF[(144.32±26.02)%比(122.75±25.33)%]、Fn水平[(0.40±0.04)g/L比(0.36±0.02)g/L]、血清TNF-α[(10.38±2.62)pg/ml比(8.15±2.03)pg/ml]、MCP-1水平[(64.13±12.76)pg/ml比(50.96±11.42)pg/ml]降低更显著(P均<0.01)。两组治疗总有效率无显著差异(P=0.435)。结论:单硝酸异山梨酯缓释片联合EECP可显著改善UAP患者心肌缺血,缓解症状,保护内皮细胞,降低炎症因子水平。Objective:To study therapeutic effect of isosorbide mononitrate sustained-release tablets combined enhanced external counterpulsation(EECP)on unstable angina pectoris(UAP).Methods:A total of 112 UAP patients treated in our hospital were randomly and equally divided into medication group(received isosorbide mononitrate sustained-release tablet based on routine treatment)and combined treatment group(received EECP based on medication group).Both groups were treated for 28 d.Seattle angina questionnaire(SAQ)score,ST segment depression degree,plasma levels of von Willebrand factor(vWF)and fibronectin(Fn),and serum levels of tumor necrosis factor(TNF)-αand monocyte chemoattractant protein(MCP)-1 before and after treatment and total effective rate was compared between two groups.Results:Compared with medication group after treatment,there was significant rise in SAQ score[(74.94±10.22)scores vs.(80.33±9.90)scores],and significant reductions in ST segment depression degree[(0.23±0.12)mV/week average vs.(0.11±0.06)mV/week average],plasma levels of vWF[(144.32±26.02)%vs.(122.75±25.33)%]and Fn[(0.40±0.04)g/L vs.(0.36±0.02)g/L]and serum levels of TNF-α[(10.38±2.62)pg/ml vs.(8.15±2.03)pg/ml]and MCP-1[(64.13±12.76)pg/ml vs.(50.96±11.42)pg/ml]in combined treatment group(P<0.01 all).There was no significant diffidence in total effective rate between two groups(P=0.435).Conclusion:Isosorbide mononitrate sustained-release tablets combined EECP can significantly improve myocardial ischemia,relieve symptoms,protect endothelial cells and reduce levels of inflammatory factors in patients with unstable angina pectoris.
