摘  要:目的探讨冠心病患者血液脂蛋白相关磷脂酶A_(2)(Lp-PLA_(2))与超敏C反应蛋白(hs-CRP)及D-二聚体的相关性。方法选取2018年1—12月收治的120例冠心病患者(研究组)与同期120名健康体检者作为研究对象(对照组)。测定患者血清Lp-PLA_(2),hs-CRP、D-二聚体、心肌损伤标志物肌钙蛋白(TnI)、肌红蛋白(Myo)、血清低密度脂蛋白胆固醇(LDL-C)、肌酸激酶MB(CK-MB)水平,比较两组上述检测指标水平,分析研究组各项指标相关性,比较不同心功能级别患者与Lp-PLA_(2)、hs-CRP、D-二聚体水平。结果研究组Lp-PLA_(2)、hs-CRP、D-二聚体、TnI、Myo、LDL-C、CK-MB水平均高于对照组,差异有统计学意义(P<0.05)。Spearman相关性分析显示,Lp-PIA_(2)与LDL-C、hs-CRP、D-二聚体呈正相关(P<0.05)。D-二聚体与心功能级别、hs-CRP呈正相关(P<0.05)。hs-CRP与TnI、Myo、CK-MB呈正相关(P<0.05)。Lp-PLA_(2)、hs-CRP、D-二聚体随着患者心功能等级增加,水平呈递增趋势(P<0.05)。Spearman相关性分析显示,研究组Lp-PLA_(2)、hs-CRP、D-二聚体与心功能等级呈正相关(P<0.05)。结论D-二聚体与hs-CRP及心肌损伤标志物密切相关,表明炎症与血栓和心肌损伤存在相关性。心功能级别低的患者更易出现Lp-PLA_(2)、hs-CRP、D-二聚体水平升高。对于冠心病患者,Lp-PLA_(2)、hs-CRP、D-二聚体检测是诊断冠心病的重要参考指标。Objective To investigate the correlation between blood lipoprotein associated phospholipase A_(2)(Lp-PLA_(2)),high-sensitivity C-reactive protein(hs-CRP)and D-dimer in patients with coronary heart disease.Methods 120 patients with coronary heart disease(study group)and 120 healthy controls during the same period from January to December 2018 were selected as the research subjects(control group).Determination of serum Lp-PLA_(2),hs-CRP,D-dimer,myocardial injury markers troponin(TnI),myoglobin(Myo),serum low-density lipoprotein cholesterol(LDL-C),creatine kinase MB(CK-MB)levels were compared between the two groups,and the correlation of each index in the study group was analyzed,and the levels of Lp-PLA_(2),hs-CRP and D-dimer in patients with different cardiac function grades were compared.Results The levels of Lp-PLA_(2),hs-CRP,D-dimer,TnI,Myo,LDL C,and CK-MB in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Spearman correlation analysis showed that Lp-PLA_(2)was positively correlated with LDL-C,hs-CRP and D-dimer(P<0.05).D-dimer was positively correlated with cardiac function level and hs-CRP(P<0.05).hs-CRP was positively correlated with TnI,Myo and CK-MB(P<0.05).The levels of Lp-PLA_(2),hs-CRP and D dimer showed an increasing trend with the increase of patients'cardiac function grades(P<0.05).Spearman correlation analysis showed that Lp-PLA_(2),hs-CRP,D-dimer were positively correlated with cardiac function grade in the study group(P<0.05).Conclusion D-dimer is closely related to hs-CRP and myocardial injury markers,indicating that inflammation is associated with thrombosis and myocardial injury.Patients with low cardiac function were more likely to have elevated levels of Lp-PLA_(2),hs-CRP,and D-dimer.For patients with coronary heart disease,the detection of Lp-PLA_(2),hs-CRP,and D-dimer is an important reference index for the diagnosis of coronary heart disease.
