摘　　要：目的探究内固定方法治疗不稳定型老年转子间骨折的临床疗效及对患者术后并发症的影响。方法选取2018年4月至2019年12月本院收治的80例不稳定型老年转子间骨折患者作为研究对象,根据治疗方法不同分为常规组与研究组,每组40例。常规组采用近端髓内钉(PFN)内固定治疗,研究组采用股骨近端抗旋髓内钉(PFNA)内固定治疗,比较两组临床疗效、围手术期指标及术后并发症发生情况。结果研究组治疗总有效率高于常规组,骨折愈合时间短于常规组,术中出血量少于常规组,差异有统计学意义(P<0.05)。两组术后并发症发生率、手术时间比较差异无统计学意义。结论PFNA治疗不稳定型老年转子间骨折效果显著,可减少术后并发症的发生,手术时间短,且术后出血量少,术后骨折愈合更快,值得临床推广应用。Objective To investigate the clinical efficacy of internal fixation in the treatment of unstable elderly patients with intertrochanteric fractures and its influence on postoperative complications.Methods 80 unstable elderly patients with intertrochanteric fractures who were treated in our hospital from April 2018 to December 2019 were selected as the research subjects,and they were divided into routine group and study group ac-cording to different treatment methods,with 40 cases in each group.The routine group was treated with proximal femoral nail(PFN)internal fixa-tion,and the study group was treated with proximal femoral nail anti-rotation(PFNA)internal fixation.The clinical efficacy,perioperative indicators and postoperative complications were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the routine group,the fracture healing time was shorter than that in the routine group,and the intraoperative blood loss was less than that in the routine group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications and operation time between the two groups.Conclusion PFNA has significant effect in the treatment of unstable elderly patients with intertrochanteric fractures,which can reduce the occurrence of postoperative complications,shorten the operation time,reduce postoperative bleed-ing,and achieve faster postoperative fracture healing.
